大黄治疗妊娠高血压综合征的临床观察

摘　　要：目的:探讨大黄对妊娠高血压综合征(PIH)的治疗作用及其作用机理。方法:治疗组口服制大黄和心痛定,对照组单服心痛定,并观察血脂、肾功能、凝血、纤溶及免疫等指标的变化。结果:治疗组(1)血甘油三酯(TG)、低密度脂蛋白胆固醇(LDLC)明显下降,高密度脂蛋白胆固醇(HDLC)明显升高;(2)血浆纤维结合蛋白(FN)、纤溶酶原活性抑制因子(PAI)明显下降,抗凝血酶Ⅲ(ATⅢ)无明显变化;(3)血β2微球蛋白(β2MG)、尿N乙酰βD葡萄糖苷酶(NAG)及白细胞介素6(IL6)明显下降;(4)外周血CD8明显升高,CD4/CD8、循环免疫复合物(CIC)明显下降;上述指标与对照组比较均有显著性差异。新生儿体重治疗组显著重于对照组,产后出血显著少于对照组。结论:大黄能显著减轻PIH血管内皮细胞损伤,调节免疫平衡,是治疗PIH比较有效的药物。Objective: To study the effect of prepared rhubarb in treating patients with pregnancy induced hypertension (PIH) and its therapeutic mechanisms. Methods: Prepared rhubarb and nifedipine were given to the study group, while the nifedipine was given to the control group alone. The blood lipid, renal function, blood coagulation, fibrinolysis and immunological parameters were monitored. Results: In the study group, (1)Levels of triglycerides (TG) and low density lipoprotein cholesterol (LDL C) decreased and high density lipoprotein cholesterol (HDL C) level increased significantly; (2)Plasma level of fibronectin (FN) and plasminogen activator inhibitor (PAI) lowered significantly, plasma antithrombin (AT Ⅲ) level unchanged; (3)Serum β 2 microglobulin (β 2 MG) level and urinary level of N acetyl β D glucosaminidase (NAG) and interleukin 6 (IL 6) decreased significantly; (4)CD 8 of blood increased, CD 4/CD 8 and circulating immune complex (CIC) of blood reduced obviously. All of the above mentioned parameters in the study group were significantly different from that of the control group. Conclusion: The prepared rhubarb could reduce the vascular endothelial cell′s damage significantly and alter the immune balance, which is effective in treating the PIH.
