颈椎骨折脱位并海水淹溺性肺水肿的治疗

摘　　要：目的:分析颈椎骨折并海水淹溺性肺水肿的治疗要点。方法:总结我院于2001年～2005年收治的6例颈椎骨折脱位并海水淹溺性肺水肿的患者资料,正压通气给氧、甲基强的松龙、东莨菪碱、纳洛酮、NO等治疗肺水肿,颅骨牵引、减压融合内固定治疗颈椎骨折。结果:死亡1例,5例未遗留肺部后遗症,颈椎骨折脱位全部愈合,截瘫指数恢复1-2级。结论:海水淹溺需警惕并存颈椎损伤,治疗应在抢救生命的同时尽早、简单而有效的治疗颈椎骨折,最大限度恢复患者功能。Objective：Analysis the therapy points of cervical fracture-dislocation with lung edema by seawater.Methods：Summarize the 6 cases of cervical fracture-dislocation with lung edema by seawater from 2001-2005.Lung edema was treated by Oxygen,Methylprednisonlone,Scopolamine,Naloxone.Cervical fracture and dislocation was treated by skull traction and surgery of decompression,confusion and internalfixation.Results：One patient died as a result of respiratory failure,no sequelae was left in other 5 patients,whose cervical fracture were healed,and paraplegia index improved by 1-2 degrees.Conclusion：We must look out cervical fracture-dislocation in drowning patients by seawater.As the same time of the salvage,we must treat the cervical fracture-dislocation with simple and efficient methods,recover the patients’ function maximumly.
