摘  要:目的探讨呼气末正压(PEEP)对慢性阻塞性肺病(chronic obstructive pulmonary diseases,COPD)急性加重期合并呼吸衰竭机械通气患者血流动力学的影响。方法运用无创心肺监测仪(NICO)监测12例COPD急性加重期合并呼吸衰竭机械通气病人在不同水平PEEP(0~20cmH2O)时的心输出量(CO)和心脏指数(CI)值,观察不同水平PEEP对血液动力学的影响。结果 PEEP≥12cmH2O时患者的CO及CI与PEEP≤4cmH2O时有显著性差异(P<0.01,P<0.05)。结论相对容量恒定时,慢性阻塞性肺病急性加重期合并呼吸衰竭机械通气患者PEEP<12cmH2O时血液动力学稳定,PEEP≥12cmH2O时对患者血液动力学可能有一定影响。Objective To investigate the hemodynamic effects of different positive end expiratory pressure levels were used in the patients with chronic obstructive pulmonary disease treated with mechanical ventilation.Methods Cardiac output(CO) and cardiac index(CI) of 12 patients with chronic obstructive pulmonary disease treated with mechanical ventilation of different selected PEEP were measured with non-invasive cardiac output(NICO).Results There were significant difference among patients measurement data about CO and CI when PEEP≥12cmH2O and PEEP≤4cmH2O.Conclusion In the relatively constant lung volume of the patients with chronic obstructive pulmonary disease treated with mechanical ventilation,the hemodynamics is stable when PEEP12cmH2O,however,the hemodynamic effects may be affected when PEEP≥12cmH2O.
