Stanford B型主动脉夹层腔内隔绝术的围手术期护理

摘　　要：目的探讨覆膜血管内支架置入治疗Stanford B型主动脉夹层围手术期的护理。方法对20例行介入术的患者,术前给予心理疏导、健康教育、营造良好的手术氛围。术后监测生命体征、肢体血运、神经症状等。结果支架置入全部成功,无死亡病例。3例破口位置距左锁骨下动脉小于0.5cm,支架置入术后封堵左锁骨下动脉,但经保守治疗无肢体缺血症状。结论在覆膜支架置入术的治疗中,积极主动地针对性观察及护理是手术成功重要保障。Objective To investigate the perioperative nursing of stent-graft placement for Stanford B aortic dissection.Methods The psychological guidance was applied in 20 patients treated by interventional operation before surgery which included explaining the method and purpose of operation,building a good surgical atmosphere,postoperative monitoring vital signs,body blood supply,neurological symptoms,etc.Results The placement of stents were successful with no death.Because distance between the laceration and left subclavian artery（LSA）0.5cm,the stent-graft blocked LSA in 3 case.And after the treatment of affected limb immobilization,rest and anticoagulant,the ischemic symptom did not occur in affected limb.Conclusion The perioperative nursing is important for stent-graft placement.
