慢性乙型肝炎患者血清脂联素水平与肝纤维化、炎症程度的相关性研究

摘　　要：目的探讨血清脂联素水平在慢性乙型肝炎(C H B)患者与肝纤维化及炎症进展的关系。方法对35例C H B患者及24例健康者血清脂联素水平行酶联免疫吸附实验(ELISA)法测定,并与肝脏纤维化分期及炎症分级进行统计学分析。将肝纤维化分期S2及S2以下患者确定为轻度肝纤维化组,S2以上确定为重度肝纤维化组。炎症分级G 2及G 2以下患者为轻度炎症组,G 2以上患者为重度炎症组。结果①轻度及重度肝纤维化组患者血清脂联素水平显著低于正常对照组(P<0.05);②轻度及重度炎症组患者血清脂联素水平显著低于正常对照组(P<0.05);③在炎症相同情况下(G 3)轻度、重度肝纤维化组脂联素水平差异无显著性(P>0.05)。结论在C H B患者血清脂联素水平变化受到肝脏纤维化及炎症两方面的影响,与肝脏炎症程度的相关性更为密切。【Objective】 Adiponectin have gained attention in the pathogenesis of fibrogenesis in various chronic liver diseases.However,the roles of adiponectin in the progression of hepatic fibrosis and inflammation in patients with chronic hepatitis B virus（HBV） infection are poorly understood.The purposes of this study are to evaluate possible associations of serum levels of adiponectin with progression of hepatic fibrosis and inflammation in patients with chronic HBV infection.【Methods】 Serum adiponectin concentration was measured by an enzyme-linked immunosorbent assay.Subjects included in this study are 35 patients with chronic hepatitis B（CHB） and 24 normal controls（NC）.Histologic activity index（HAI） of inflammation and Knodell fibrosis were scored in 35 patients with CHB,patients with and under G2 stage were included as mild inflammation and patients above G2 stage were included as advanced inflammation,patients with and under S2 stage were included as mild fibrosis and patients above S2 stage were included as advanced fibrosis.【Results】 Serum levels of adiponectin in patients with mild fibrosis,advanced fibrosis were significantly lower than those in NC.Serum levels of adiponectin in patients with mild inflammation,advanced inflammation were significantly lower than those in NC.We found that in patients with the same histologic activity index,there was no significant difference between patients with mild and advanced fibrosis.【Conclusion】 Serum levels of adiponectin decrease as hepatic inflammation and fibrosis progression in patients with chronic HBV infection,and our results indicate that serum levels of adiponectin correlate more closely with the degrees of hepatic inflammation other than the stages of hepatic fibrosis in patients with chronic HBV infection.
