非小细胞肺癌中医证候分析

摘　　要：目的调查非小细胞肺癌(NSCLC)的中医证候分布,并探讨其与组织病理类型、临床分期的关系。方法选取经组织病理学或细胞学确诊为NSCLC患者125例,自拟调查表进行中医证候调查,并分析不同组织病理类型及不同临床分期患者中医证候分布情况。结果单一证候5例占4%;复合证候115例占92%;无症状5例占4%。2个单一证候相兼的病例为15例占12.0%,3个及3个以上单一证候相兼并存的病例有100例占80.0%。不同组织病理类型患者中医证候分布差异无统计学意义(P>0.05);不同临床分期患者中医证候分布差异有统计学意义(P<0.05)。结论气虚、痰凝、血瘀是非小细胞肺癌最常见的中医证候;Ⅰ、Ⅱ期NSCLC患者以实证居多,Ⅲ、Ⅳ期患者以虚实夹杂、虚证为主;中医证候与组织病理之间无明显相关性。Objective To investigate the distribution of TCM syndromes of non-small cell lung cancer（NSCLC） and study its relation with histopathological types and clinical stages.Methods Totally 125 NSCLC patients diagnosed with histopathology or cytology were investigated with self-made questionnaire on TCM syndromes.The distribution of TCM syndromes in different histopathological types and clinical stages was analyzed.Results There were only 5 patients,accounted for 4%,menifesting as single syndrome,and there were 115 patients,occounted for 92%,having composite syndromes,and there were 5 patients,being 4%,having no symptoms.There were 15 patients,accounted for 12.0%,having two single syndromes together,and there were 100 patients,counted for 80.0%,having three or more syndromes complicated with each other.In the different histopathological types,there was no statistical difference in the distribution of TCM syndromes（P〉0.05）.In the different clinical stages,there was statistical difference in the distribution of TCM syndromes（P〈0.05）.Conclusion Qi deficiency,phlegm stagnation,and blood stasis are the most common TCM syndromes of NSCLC patients.The patients of stages Ⅰ and Ⅱare mostly having excess syndromes,and those of stages Ⅲ and Ⅳ are mainly having complicated syndromes of deficiency-excess and deficiency syndromes.There is no significant correlation between the TCM syndromes and histopathology.
