摘 要:目的:探讨糖尿病患者视网膜动脉、视网膜静脉直径对心血管疾病发生的预测价值。方法:纳入72例糖尿病患者,其中23例为心肌梗死组,其余49例未发生心肌梗死的为对照组。对所有患者计算视网膜中央动脉直径等效值(central retinal arteriolar equivalents,CRAE)和视网膜中央静脉直径等效值(central retinal venular e-quivalents,CRVE),以此分别代表视网膜动脉和视网膜静脉的平均直径。通过问卷调查和抽取静脉血获取其他心血管危险因素。研究2组患者CRAE和CRVE的差异性,以及其他心血管危险因素对视网膜血管直径的影响。结果:心肌梗死组患者CRAE、CRVE均较对照组缩窄;与CRAE有相关性的其他指标有年龄、性别、血压、体质指数、吸烟史、高血压史和LDL-C、HDL-C;与CRVE有相关性的有糖尿病病程、吸烟史、高血压史和糖化血红蛋白。结论:视网膜血管直径可在一定程度上预测糖尿病患者心血管疾病发生风险,但需考虑到其他心血管危险因素对于视网膜血管直径的影响。Objective:To investigate the prediction value of the diameter of the retina vessels of the diabetic patients for the incidence of cardiovascular diseases. Method:The photos of the retina from which the central retinal arteriolar equivalents(CRAE) and central retinal venular equivalents(CRVE) were calculated to represent the average diameters of the central retinal artery and the central vein respectively,were taken from 72 diabetic patients for,23 of who were suffered with myocardial infarction(MI),and the other 49 without MI.The other cardiovascular risk factors were obtained through the questionnaire and venous blood.The differences of CRAE and CRVE between the two groups and the influences of the other risk factors on the CRAE and CRVE were calculated. Result:The CRAE and CRVE were smaller in the patients with MI than those without MI.Other factors that affected the CRAE were age,sex,systolic pressure,diastolic pressure,body mass index,current smoking,hypertension,low density lipoprotein and high density lipoprotein,and ones that affected the CRVE were course of diabetes,current smoking,hypertension and HbA1c. Conclusion:The diameters of retinal vessels can partly predict the incidence of cardiovascular diseases in the diabetes patients,but it should be considered that other cardiovascular risk factors can influence the diameters of retinal vessels.
