摘  要:目的:观察黄芪注射液对2型糖尿病患者外周血巨噬细胞过氧化物酶体增殖物激活受体γ(PPARγ)mRNA表达的影响。方法80例2型糖尿病患者随机分为DM1组(40例):维持原治疗不变;DM2组(40例):加用黄芪注射液治疗;正常对照组:30例健康体检者。3组治疗前,DM1和DM2组治疗后的外周血经分离培养其单核巨噬细胞,采用RT-PCR技术扩增PPARγ基因片段,检测其表达水平,并测其空腹血糖(FBG)、空腹胰岛素(FINS)、甘油三酯(TG)、总胆固醇(TC),以稳态模式评估(HOMA)指数作为评价胰岛素抵抗(IR)指标。结果治疗前DM1及DM2组的PPARγmRNA表达比正常组显著降低(P<0.05)。治疗后DM2组的FBG、FINS、HOMA指数、TC较治疗前明显降低,而PPARγ的mRNA表达则明显增加(P<0.05)。治疗后DM1组和DM2组比较,后者HOMA指数及TC水平较前者降低,PPARγmRNA较前者升高。结论:黄芪能改善2型糖尿病患者胰岛素抵抗,调节脂质代谢,而通过激活PPARγ是其可能机制,从而达到防治糖尿病以及动脉粥样硬化的作用。Objective:To investigate the effect of Astragalus on peroxisome proliferator-activated receptorγ(PPARγ) mRNA expression in macrophage with Type2 Diabetic Mellitus.Methods: 80 Type2 Diabetic Mellitus were divided into 2 groups randomly,DM1group(40 patients):retain the same treatment;DM2group(40patients):add Astragalus;and 30 healthy man as control.we separated the monocyte-macrophage cells,extracted the RNA from the cells,amplificated PPARγ gene fragment by reverse transcriptase-PCR and than Detected the expression level by semi-quantitative fluorescent method on the 3groups Before experiment,DM1and DM2groups after treatment.The fasting blood glucose(FBG),insulin(FINS),total cholesterol(TC),triglyceride(TG),insulin homeostasis model assessment(HOMA) index were also measured.Results:Before treatments,the PPARγ mRNA expression were more lower in DM1and DM2groups.After add Astragalus,the PPARγ mRNA expression was significantly increased in DM2 group,meanwhile the FBG,FINS,HOMA index and TC were decreased obviously.To compare the DM1and DM2groups after treatment,the latter had higer PPARγ mRNA expression and lower TC and HOMA index.Conclusion:Astragalus can improve insulin sensibility,adjust lipid,which may be associated with its effects in increasing the expressions of PPARγ,and than play a part in curing the diabetes and artherosclerosis.
