摘  要:目的探讨库存红细胞(RBCs)输注对受者炎症反应的作用。方法将RBCs在保存1、21、35d(以下用D1、D21、D35表示)后分别分离其上清,-20℃冻存备用。将冻存上清分别与健康受者外周血单个核细胞(PBMC)体外培养24h后,加入脂多糖(LPS)再培养24、48h后收集培养细胞的上清冻存待检。用ELISA分别检测细胞培养上清中细胞因子IL-1β、IL-6、IL-17、TNF-α、TGF-β及GM-CSF浓度的变化。结果在LPS诱导下,随保存时间的延长,库存红细胞可增强PBMC分泌TNF-α和IL-6;而TGF-β逐步减少。结论随悬浮红细胞保存时间的延长,可增强受者PBMC分泌TNF-α和IL-6含量,推测输注保存时间较长的红细胞可能增强受者的炎症反应,这为进一步阐明临床严重创伤患者大量输血后的相关免疫应答提供了理论依据。Objective To study the effect of banked red blood cells(RBCs) transfusion with various duration of blood storage on the inflammation in recipients.Methods The supernatants were isolated from the banked RBCs at different time points(day 1,day 21 and day 35) respectively,and incubated with peripheral blood mononuclear cells(PBMCs) from healthy donors for 24 h,then with lipopolysaccharide(LPS) for an additional 24 h culture.Then cells′ supernatants were collected after 48 hours of incubation and analyzed for IL-1β,IL-6,IL-17,TNF-α,TGF-β and GM-CSF by ELISA.Results The level of TNF-α and IL-6 secreted from PBMCs induced by LPS was increased significantly after transfusion of banked RBCs,related to the duration of blood storage.Meanwhile the TGF-β was decreased gradually.Conclusion The level of TNF-α and IL-6 secreted from PBMCs is increased significantly by the RBCs transfusion along with the extension of blood storage.The inflammatory response in recipients upon LPS induction is enhanced by the RBCs transfusion with the elongation of the banked RBCs duration.The results shed light on the mechanism underlying the transfusion-related immunomodulation in trauma.
