摘 要:目的探讨二维高频超声、彩色多普勒超声和超声弹性成像在浅表软组织肿物良恶性鉴别中的价值。方法对83例浅表软组织肿物患者(共计108个病灶)进行超声检查,全部资料均经术后病理证实(良性肿物56个,恶性肿物52个),对其中69个肿物做了超声弹性成像评分。常规超声检查了解肿物的位置、大小、形态、边界、内部回声、与周围组织的关系;用彩色多普勒超声重点观察肿物周围及内部血流分布;然后通过弹性成像评分判定肿物质地软硬。分析图像,总结上述结果在鉴别良恶性病变中的应用价值。结果良、恶性浅表软组织肿物所达组织层次、大小、血流分布、弹性成像评分的差异均有统计学意义(P<0.05)。以血流分布多作为判断恶性的标准,其敏感性为69.2%,特异性为80.4%,准确性为75.0%。以超声弹性成像评分≥4分作为判断恶性的标准,其敏感性为72.7%,特异性为80.6%,准确性为76.8%。结论二维高频、彩色多普勒超声及弹性成像的联合应用有助于对浅表软组织肿物的良恶性鉴别。Objective To explore the value of two-dimensional high-frequency ultrasonography, color Doppler imaging and ultrasonic elastography(UE) in differentiating the benign and malignant superficial soft tissue masses. Methods Two-dimensional high-frequency and color Doppler ultrasonography were performed on 83 patients with 108 superficial soft tissue masses. All of the lesions were confirmed by pathological examination. There were 56 benign masses and 52 malignant masses. Elastography was performed on 69 masses. The depth, size, edge,echoes, blood flow distribution and elasticity scores of the masses were evaluated. The value of the above results in differentiating benign and malignant lesions was discussed. Results The differences between benign and malignant superficial soft tissue masses of depth, size, blood flow distribution and elasticity scores were of statistical significance(P〈0.05). The sensibility, specificity and accuracy for diagnosing malignant lesions were respectively 69.2%, 80.4% and 75.0 by color Doppler imaging, while 72.7%, 80.6% and 76.8% by ultrasonic elastography. Conclusions Combined application of high-frequency ultrasound, color Doppler imaging and ultrasonic elastography is helpful to differentiate benign from malignant superficial soft tissue masses.
