实时三维超声心动图早期诊断心肌缺血的临床研究

摘　　要：目的探讨实时三维超声心动图17节段时间容积曲线参数对早期冠心病心肌缺血诊断的价值。方法 (1)冠心病组60例,男37例,女23例,年龄37～82岁,冠脉造影至少1支血管狭窄≥50%。(2)对照组25例,男14例,女11例,年龄39～75岁,为冠脉造影正常者。所有入选者行实时三维超声检查。结果与对照组比较,冠心病组17节段容积-时间曲线参数指标——左室16节段、12节段、6节段达最小收缩容积的时间标准差(Tmsv 16-SD、Tmsv 12-SD、Tmsv 6-SD)、最大时间差值(Tmsv 16-Dif、Tmsv 12-Dif、Tmsv 6-Dif)及各自的经心动周期的校正值(Tmsv 16-SD%、Tmsv 12-SD%、Tmsv 6-SD%)、(Tmsv 16-Dif%、Tmsv 12-Dif%、Tmsv 6-Dif%)显著增加(P<0.05)。结论实时三维超声心动图17节段容积时间曲线参数对早期诊断冠心病能提供更多的信息。Objective To apply the seventeen segments time-volume curves of the real-time three- dimensional echocardiography to evaluate the early diagnois of myocardial ischemia in coronary heart disease. Methods Sixty patients with coronary heart disease were enrolled（37 men, 23 women, aged 37-82 years）, at least one coronary artery stenosis（〉50 %） by angiocardiography. Twenty-nine controls（14 men, 11 women, aged 39-75 years） were normal by an- giocardiography. All patients were measured by the real-time three-dimensional echocardiography. Results Compared with the normal group, the parameters（Tmsv 16-SD.Tmsv 12-SD.Tmsv 6-SD ：the time to end-sistolic minimum volume of the left 16 regional. 12 regional, 6 regional; Tmsv 16-Dif. Tmsv 12-Dif. Tmsv 6-Dif： time interval difference to end-sistolic minimum volume of the left 16 regional.12 regional,6 regional;and correction value by cardiac cycle of respective parameters ： Tmsv 16-SD%. Tmsv 12-SD%. Tmsv 6-SD%. Tmsv 16-Dif~. Tmsv 12-Dif%. Tmsv 6-Dif % ） of the seventeen segments time-volume curves of the patients in coronary heart disease were increased significantly（P-（ 0.05）. Conclusions The seventeen segments time-volume curves of the real-time three-dimensional echocardiography can provide more information for early diagnosis of coronary heart disease.
