摘  要:目的探讨CD4+CD25+CD127low/-T细胞在慢性乙型病毒性肝炎肝纤维化发病机制中的作用。方法按肝纤维化S分期将慢性乙型肝炎肝纤维化患者分为S1~S2期6例和S3~S4期7例;采用磁激活细胞分选法(MACS)提取13例患者和6例健康对照者外周血CD4+CD25+CD127low/-T细胞;分别将CD4+CD25+CD127low/-T细胞以不同比例与人肝星状细胞株(hepaticstellate cell,HSC)共培养5天,检测各组HSC的增殖及TGF-β1的分泌。结果 CD4+CD25+CD127low/-T细胞与HSC之比,当健康对照组和S1~S2期组为1.5∶1,S3~S4期组为2∶1时,HSC的增殖速度和TGF-β1分泌量均在同组不同比例中达最高值。各组同比例之间,当比例为1.5∶1时,健康对照组HSC的增殖速度显著高于S3~S4期组(P<0.05);在比例为1∶1~2∶1范围内,S1~S2期组和S3~S4期组HSC的TGF-β1分泌量远高于同比例健康对照组(P<0.05)。结论 CD4+CD25+CD127low/-T细胞可能通过自身细胞数量和功能的双重调节,影响肝星状细胞的增殖和分泌,参与了慢乙肝肝纤维化的进展。Objective To investigate the roles of CD4+CD25+CD127^low/-T cells in HBV - related liver fibrosis. Methods Fibro- sis stage was confirmed by pathohistological examination of liver section. CD4+CD25+CD127^low/-T cells were isolated respectively from peripheral blood of 6 healths,6 patients in S1 - S2 stage and 7 patients in S3 - S4 stage by MACS. We then cultured CD4+CD25+CD127^low/-T cells and human hepatic stellate cell(HSC) with different ratios respectively,and after 5 days,appraised the proliferation of HSC by CCK8 and used Elisa method to test concentration of TGF - β1 secreted by HSC. Results We discovered the strongest prolifera- tion and the highest TGF - β1 secretion of HSC when the healths and patients in S1 - S2 stage ratio of (the numbers of CD4+CD25+CD127^low/-T cells/HSC) 1.5:1 ,patients in S3 - S4 stage of 2: 1. From health control group,we also found that HSC had a stronger proliferation compared with patients in S3 - S4 stage when ratio was 1.5:1 (P 〈 0. 05 ). And the ratios ranged from 1:1 to 2: 1. There was a lower TGF -β1 secretion of HSC in health control group compared with other groups(P 〈 0.05). Conclusion Changes of numbers and function- alities of CD4+CD25+CD127^low/-T cells might promote HSC proliferation and TGF - β1 secretion and contribute to progression of HBV - related liver fibrosis.
