阿德福韦酯联合双环醇片治疗慢性乙型肝炎疗效观察

摘　　要：目的 研究阿德福韦酯联合双环醇片治疗慢性乙型肝炎的疗效.方法 将87例慢性乙型肝炎患者完全随机分为试验组（44例）和对照组（43例）.试验组口服阿德福韦酯10 mg,1次/d;同时服用双环醇片25mg,3次/d.对照组口服阿德福韦酯10 mg,1次/d.2组均连续用药48周.观察2组治疗后肝功能及病毒学指标方面的改变.结果 治疗后2组肝功能指标均有明显改善,试验组与对照组比较差异有统计学意义[ALT：（40.3±16.5）U/L比（46.7±13.1）U/L,AST：（37.5±19.4）U/L比（45.4±26.1）U/L,A/G：（1.4±0.2）比（1.3±0.3）,γ谷氨酰转移酶：（35.8±2L4）U/L比（55.2±17.3）U/L,总胆红素：（14.8±12.3）μmol/L比（16.8±11.7）μmol/L,P＜0.05或P＜0.01）].与治疗前比较,试验组和对照组治疗后HBVDNA阳性率差异均有统计学意义[试验组34.1%（15例）比100.0%（44例）,对照组32.5%（14例）比100.0%（43例）,均P＜0.01],但2组治疗后比较差异无统计学意义（P＞0.05）,血清学转换组间、组内比较差异均无统计学意义（P＞0.05）.结论 阿德福韦酯与双环醇片联合应用治疗慢性乙型肝炎,在病毒学方面取得较好疗效,在肝功能方面有较明显的改善,且安全性良好.Objective To analyze the efficacy and safety of adefovir dipivoxil combined with bicyclol in treatment of chronic hepatitis B （CHB）. Methods A total of 87 patients with CHB were randomized into experimental group（44 cases） and control group（43 cases）. The patients in the experimental group received adefovir dipivoxil orally 10 mg daily and bicyclol orally 75 mg daily for 48 weeks and control group received adefovir dipivoxil orally 10 mg daily alone for 48 weeks. The live function（serum transaminase ect）, HBV DNA, and HBeAg/antiHBe were observed before and after treatment. Results Compared with pre-treatment, the live function decreased obviously in two groups respectively, especially in the experimental group [ALT： （40.3 ± 16.5）U/L vs （46.7 ±13.1）U/L, AST： （37.5 ±19.4）U/L vs （45.4 ±26. 1）U/L, A/G：（1.4 ±0.2） vs （1.3 ±0.3）, r-GT：（35.8 ±21.4）U/L vs （55.2 ± 17.3）U/L, TBIL： （14.8 ± 12.3） μmol/L vs （16.8 ± 11.7） μmol/L, P 〈0.05, P〈0. 01）]. HBV DNA negative conversion rate also decreased （P 〈 0.01）, while the comparison between the two group after the treatment was not significantly different. There were no statistical differences between the two groups or per-treatment and treatment regarding with HBeAg seroconversion rate. Conclusion Administration of adefovir dipivoxil combined with bicyclol is effective and safe in the treatment of CHB, especially in HBV DNA negative conversion and the live function.
