摘  要:目的探讨高尿酸血症与IgA肾病临床病理的关系,及影响高尿酸血症的因素。方法将经肾活检明确诊断为原发性IgA肾病患者92例,对其临床及病理资料进行回顾性分析。结果①IgA肾病肾功能不全患者高尿酸血症的发病率(40.54%)明显高于肾功能正常患者(20%),P<0.05;②与高尿酸血症患者与血尿酸正常患者在肾功能不全与肾功能正常组中分别进行比较,平均动脉压,24h尿蛋白定量、血肌酐、尿素氮、血清白蛋白、甘油三脂、总胆固醇、血清IgA及体重指数差异无统计学意义(P>0.05);但根据肾脏Katafuch i半定量评分显示在肾功能正常组和肾功能不全组中,高尿酸血症患者的肾脏总积分、肾小球积分、肾小管间质积分均高于血尿酸正常组患者,差异有统计学意义(P<0.05);③多元线性逐步回归分析表明,肾小管间质积分、体重指数、甘油三酯、平均动脉压是血尿酸独立危险因素。结论高尿酸血症与IgA肾病肾脏病理损害密切相关,严格控制高尿酸血症及影响因素,可减低高尿酸血症对肾脏的损害,延缓IgA肾病的进展。Objective To explore the effect of hyperuricaemia on clinicopathology and the influence factors of IgA Nephropathy.Methods A total of 92 patients with IgA nephropathy diagnosed by renal biopsy in our hospital were enrolled in this study.The data included clinicopathological and the influence factors of IgA nephropathy(IgAN) patients were retrospectively analyzed.Results The incidence of hyperuricaemia in the group of abnormal renal function was higher than that in the group of normal renal function(P0.05).There were no significant in albumin,triglyceride,cholesterol,serum of IgA and BMI between patients of differences of mean arterial pressure,24 hour proteinuria,Scr,BUN,serum of hyperuricaemia and normal uric acid in the group of abnormal renal function and normal renal function respectively.According Katafuchi semi-quantitative pathological scoring,the total score,glomerulus score,tubular score were significantly different in two groups.The multi-factor stepwise regression analysis indicated that there was linear between the uric acid with the tubular score,BMI,triglyceride and Mean arterial pressure.Conclusion The uric acid is related to the renal injury in human IgAN,so it is important to pay more attention to lower the level of serum uric acid and control the influence factors in order to improve long-term survival of kidney.
