乌拉地尔对高血压患者全麻拔管期脑电双频指数的影响

摘　　要：目的观察乌拉地尔对高血压患者拔管期脑电双频指数的影响。方法将60例患有原发性高血压病的择期腹部手术全麻患者随机分为D组和W组。2组患者全麻诱导和维持相同,术毕在脑电双频指数(BIS)值为70时,D组给予生理盐水,W组给予乌拉地尔。观察并记录2组给药前、拔管时、拔管后2 min、拔管后5 min的收缩压(SBP)、舒张压(DBP)、心率(HR)和BIS。结果 D组在拔管时、拔管后2 min、拔管后5 min SBP、DBP、HR、BIS与用药前比较均显著升高;与用药前相比,W组在拔管时、拔管后2 min、拔管后5 min SBP、DBP下降,HR无明显变化,BIS升高程度显著低于对照组。结论乌拉地尔能抑制高血压患者围拔管期的心血管反应,抑制BIS值升高。Objective It is to observe the effect of urapidil on the bispectral index in patients with hypertension in the period of tracheal extubation under general anesthesia.Methods Sixty patients with hypertension who were undergoing abdominal surgery under general anesthesia were randomly divided into group D and group W.The scheme of induction and maintenance of anesthesia of both groups were identical.When BIS was 70 by the end of the operation,patients in group D were given physiological saline and in patients group W were given urapidil.The SBP,DBP,HR,BIS before drug infusion,extubation and at 2,5 min after extubation were observed and recorded in both groups.Results Compared with those before drug infusion the SBP,DBP,HR,BIS of patients in group D increased significantly at the tracheal extubation and 2,5 min after the extubation.And the SBP,DBP of the patients in group W at the tracheal extubation and at 2,5 min after extubation were remarkably lower than those before drug infusion,HR had no marked alternation than before drug infusion,BIS increased degree was significantly lower than that in control group.Conclusion Urapidil can restrain cardiovascular responses of patients with hypertension in the period of tracheal extubation,but also inhibit BIS increasing.
