亚高原地区高血压与颈动脉病变关系的探讨

摘　　要：目的探讨亚高原地区原发性高血压与颈动脉粥样硬化之间的关系,为高血压及其脑血管并发症防治提供参考依据。方法从2009年4月～2010年6月昆明企事业单位来我院体检中心例行体检的人群中,分别随机抽取符合高血压诊断的1000名体检者设为高血压组,非高血压者673名设为对照组。对两组人员进行颈动脉超声检查,记录颈动脉内-中膜(IMT)厚度、斑块情况、有无狭窄及狭窄程度,并对结果进行分析。结果高血压组IMT增厚、斑块检出率、颈动脉狭窄和轻中度狭窄检出率均明显高于对照组(P<0.05)。结论亚高原地区高血压是颈动脉病变的重要危险因素,应对高血压患者进行经常性健康保健知识教育,提高对高血压及其并发症的知晓率,提高降压治疗效果监测及定期超声检查的依从性,配合积极的综合性治疗,改善预后。Objective To explore the relationship between essential hypertension（EH） and carotid atherosclerosis,and to provide references for the prevention and cure of EH and its complications.Methods Among those from enterprises and government offices in Kunming City receiving routine physical examination in our hospital between April 2009 and June 2010,1000 subjects who met the diagnostic criteria of EH were randomly selected and served as hypertension group.Other 673 subjects without hypertension were selected as control group.Intima-media thickness（IMT）,plaque,stenosis and stenotic degree of carotid artery were detected by carotid ultrasound,and the results were analyzed subsequently.Results The detection rates of thickened IMT,plaque and carotid stenosis as well as mild/moderate stenosis were significantly higher in hypertension group than those in control group.Conclusion Hypertension is a critical risk factor of carotid atherosclerosis in sub-plateau.Frequent health care education,regular monitoring of blood pressure and ultrasound check-up combined with comprehensive therapies should be done for hypertension patients to prevent or delay the development of cerebrovascular lesions and improve its prognosis.
