B型利钠肽检测对急性心肌梗死治疗效果的评价

摘　　要：目的:评价B型利钠肽在急性心肌梗死患者1周内的动态变化对急性心肌梗死患者近期心脏功能的预测价值。方法:首次发生的ST段抬高急性心肌梗死患者60例,按治疗方法随机分为介入治疗组与对照组(药物治疗组),患者入院后即刻、入院后24小时以及入院后1周,共3次抽取静脉血,床旁即刻测定B型利钠肽含量,同时检测肌酸激酶同工酶(CKMB)、肌钙蛋白(cTnI),超声心动图同步评价左室形态、心功能的变化及室壁节段性运动评分。随访3月,再次行超声心动图检查,重新评价。结果:两组患者入院1周时的B型利钠肽值与3月后的左室射血分数值之间存在负相关关系,介入组偏相关系数-0.863(P<0.001),对照组为-0.805(P<0.001)。3月后的介入组左室射血分数高于对照组(P<0.05)。结论:动态检测B型利钠肽可用于急性心肌梗死患者近期预后的评估。Objective： To observe the clinical application value of dynamic measurement of B-type natriuretic peptide（BNP） in patients with acute myocardial infarction（AMI） and combine same pace echocardiogram to evaluate the predicting value of short-term heart function of the patients with AMI.Methods： Sixty consecutive patients with first occurrence ST segment elevated myocardial infarction were randomly divided into two groups as follows： percutaneous coronary intervention（PCI）group and control group.The blood samples of BNP were measured on hospital admission、24 hours later and a week later,and plasma enzymes and cTnI was measured at the same time.At the same pace,echocardiogram was performed to evaluate the shape of left ventricular 、change of heart function and regional wall motion score integration（WMSI）,and left ventricular form and heart functional change were reevaluated by echocardiogram after 3 months following-up.Results： There was negtive correlation between BNP on admission a week later and left ventricular ejection fraction（LVEF） on admission 3 months later.Conclusion： Dynamic measurement of brain natriuretic peptide（BNP） of the patients with acute myocardial infarction（AMI） can be used as a factor to evaluate short-term heart function.
