摘  要:目的:探讨64层螺旋CT冠状动脉成像的临床应用价值。方法:100例临床诊断可疑冠心病患者进行64层螺旋CT冠状动脉成像,男性65例,女性35例,年龄35~78岁,平均年龄56.5岁。回顾性心电门控增强扫描,利用多层面重建(MPR)、最大密度投影(MIP)及容积再现技术(VRT)重组图像,判定冠状动脉病变情况,并与DSA对照。结果:100例患者中,左前降支、左旋支、右冠状动脉重组像评价图像质量,在心率<70次/min时,左前降支、左旋支、右冠状动脉显示较好且能满足影像学评价。MPR、MIP及VRT重组像能显示冠状动脉的所有1级、大部分2、3级以及部分4级分支。64层螺旋CT冠状动脉成像评价≥50%,冠状动脉狭窄的敏感性为94.0%,特异性为94.0%,符合率79.0%。结论:64层螺旋CT冠状动脉成像是一种无创、快速的成像方法,在多数情况下能较好地显示冠状动脉,可以做为冠状动脉病变的筛选方法。Objective: To determine the clinical application value of 64-MSCT for angiography in coronary artery.Methods One hundred patients with known or suspected coronary artery disease were studied by 64-MSCT angiography in coronary artery. There were sixty-five males and thirty-five females. The age ranged from thirty-five to seventy-eight(average,56.5). CT angiography was performed using by ECG-gated,Images were retrospectively reconstructed under different reformations multiplanar reconstruse-tion (MPR),maximum intensity projection(MIP)and volume rendering technology (VRT) and compared with conventional angioraphy. Results In the one hundred patients,for left anterior descending,left circumflex and right coronary artery,optimal image quality was assessed.With heart rate70bpm,the proportion of the left anterior descending,left circumflex and right coronary artery that could be evaluated.All of the first,most of the second and third,and parts of the fourth subsegment branches could be shown with MPR,MIP and VRT reconstruction images.The se nsitivity,specificity,accuracy of 64- MSCT coronary angiography in diagnosing(≥50%)coronary stenosis was 94.0%,94.0% and 79.0%. Conclusion 64-slice spiral CT coronary angiography is a noninvasive and fast method that allows reliable diagnosis of coronary artery stenosis with high image quality.It permits the detection of coronary artery.It might be a method of choice for patients with coronary artery disease.
