腰大池持续闭式引流治疗外伤性脑脊液漏139例临床分析

摘　　要：目的:探讨持续腰大池持续闭式引流术治疗外伤性脑脊液漏的疗效。方法:回顾性分析2001年2月至2010年12月经持续腰大池闭式引流治疗的139例外伤性脑脊液漏患者的临床资料。结果:139例中118例患者脑脊液漏治愈,平均带管时间7.3天,无严重并发症,11例合并感染者经鞘内注药治愈;14例因堵管重新置管;9例出现神经根刺激症状,拔管后症状消失;12例因复发行手术修补,4例死于严重合并症。结论:应用持续腰大池闭式引流术治疗外伤性脑脊液漏简便、安全、有效。Objective： To explore the curative effect of closed continuous lumbar subarachnoid drainage on the traumatic cerebrospinal fluid （CSF）fistula. Methods： The clinical data of 139 patients with traumatic CSF fistula treated by closed continuous drainage via a lumbar subarachnoid catheter were retrospectively analyzed from Feb 2001 to Dee 2010. Results： Among the 139 cases, 118 cases were cured. The average drain time was 7.3 days. There were no severe complications. 11 cases with intracranial infection were recovered by intrathecal injection. 14 cases required replacement because of catheter obstruction. 9 cases were seen transient lumbar nerve root irritation, and all symptoms resolved after drain removal. 12 cases were repaired by a surgical operation because of recurrence of fistula. 4 died from other severe complications. Conclusion： Closed continuous lumbar subarachnoid drainage is a simple, safe, and effective way for the treatment or prevention of CSF fistulas.
