摘　　要：目的:评价脂微球前列腺素E1治疗糖尿病足的疗效与安全性。方法:检索万方、中国知网和OVID-LWW数据库,收集以前列腺素E1(lipo-PGE1,凯时,前列地尔)为干预措施治疗糖尿病足的随机对照试验(RCT),依据文献的随机方法、分配方案隐藏、盲法、退出/失访的报告,评价纳入研究方法质量,并提取数据进行Meta分析。结果:共纳入14个RCT,包括921例糖尿病足患者。试验组为综合治疗基础上加lipo-PGE1,对照组为综合治疗或在综合治疗基础上加其它治疗药物。Meta分析结果显示,在改善糖尿病足症状和体征方面,试验组好于对照组(P<0.01),OR及其95%置信区间为5.98(4.10,8.72)。依照不同干预方法及随访时间做亚组分析,结论不变。不良反应发生方面,对照组发生率显著高于试验组(P<0.05)。结论:lipo-PGE1对糖尿病足患者的症状和体征有明显的改善作用,并能够降低不良反应的发生率。由于纳入文献的试验方法学质量较低,导致文献质量不高,尚有待开展科学规范、高质量的RCT进一步验证其疗效和安全性。Objective：To appraise the effects of Lipo-PGE1 on patients with diabetic foot.Methods：We searched the electronic databases（WANFANG,CNKI,and OVID）in order to obtain the randomized controlled trial（RCT）with interventions of Lipo-PGE1.The methodological quality of the trials was assessed by using four standards：random allocation program,allocation concealment,blind and quit.Results：Fourteen trials and 921 patients were included.Lipo-PGE1 group improving the overall efficiency ameliorated symptoms of diabetic foot better than control group（P0.01）.The effect index was 5.98（4.10,8.72）.According to different interventions and a period of treatment,we did three subgroup analysises and got the same conclusion.In the occurrence of adverse reactions,Lipo-PGE1 group was lower than control group（P0.05）.Conclusion：Applying Lipo-PGE1 could improve the overall efficiency improvement of symptoms and reduce the incidence of adverse reactions in patients with diabetic foot.However,the methodological quality of trials are very low,we need to carry out scientific,standardized and high-quality RCT,in order to further verify the efficacy and safety of the effects of Lipo-PGE1 on patients with diabetic foot.
