毒品注射致股动脉假性动脉瘤并感染的外科手术治疗

摘  要:目的探讨人工血管旁路术在毒品注射所致股动脉假性动脉瘤并感染治疗中的作用。方法 2007年3月~2011年3月,对38例毒品注射所致股动脉假性动脉瘤并感染患者的临床资料进行回顾性分析。男35例,女3例,年龄26~58岁。除1例因术前急性肺水肿死亡,1例行股动脉结扎外,余36例均进行髂股动脉人工血管旁路术及动脉瘤体切除。结果股动脉结扎1例术后因缺血坏死行截肢手术。人工血管旁路术36例术后下肢血运均重建满意,肢体保全,1例发生严重软组织感染,2例术后3~5个月人工血管闭塞合并感染予取出。1例术后1年人工血管闭塞,未予处理。结论对于毒品注射所致股动脉假性动脉瘤并感染使用人工血管旁路术重建血运是较为安全有效的治疗方法。合理的旁路路径选择、瘤腔处理、有效的抗感染和换药治疗是保障手术成功的关键。【Objective】 To explore the surgical treatment of infected femoral pseudoaneurysms(PA) caused by injection of addictive drugs.【Methods】 The clinical data of 38 cases of infected PA caused by drug injection from 2007.3.to 2011.3.were reviewed retrospectively.Of them 1 case died before surgery and 1 case received femoral artery ligation.Other 36 cases were treated with blood flow reconstruction by artificial blood vessel transplantation and aneurysm resection.【Results】 1 case received amputation for acute ischemic necrosis.In 36 cases legs were saved and the blood perfusion was satisfying after artificial blood vessel transplantation.Among them severe infection occurred in 1 case.In 2 cases artificial blood vessels were taken out for infection and occlusion 3~5 months after surgery.The vessel of 1 case was kept for occlusion without infection after 1 year after surgery.【Conclusion】 Blood flow reconstruction by artificial blood vessel transplantation and aneurysm resection is a safe and effective surgical treatment for infected femoral pseudoaneurysms caused by injection of addictive drugs.Reasonable route design,appropriate aneurysm treatment and effective anti-infection therapy are key points in the surgical treatment.
