摘  要:目的通过比较控制减压和快速减压两种不同模式治疗兔急性颅高压后脑组织中IL-1β、Nfκbp65、Bax、Caspase-3的表达及细胞凋亡水平,为控制减压模式在临床中的应用提供实验依据。方法采用硬膜外球囊加压法制备兔急性颅高压模型。新西兰白兔随机分为假手术组、快速减压组和控制减压组,观察术后颅内压的变化、CT与MRI的改变,以及IL-1β、Nfκbp65、Bax、Caspase-3的表达和凋亡细胞。结果减压后24h,控制减压组动物的脑梗死发生率(10%)低于快速减压组(35%),控制减压组动物IL-1β、Nfκbp65、Bax、Caspase-3的表达水平及细胞凋亡率均低于快速减压组。结论控制减压模式能减轻脑组织缺血再灌注损伤,降低脑梗死的发生率。Objective To provide experimental bases for the application of controlled decompression in clinical treatment by comparing different expressions of IL - 1β ,Nfκbp65 ,Bax,Caspase -3 and neuronal apoptosis in brain tissues of rabbits of acute intracranial hypertension between controlled decompression group and rapid decompression group. Methods The rabbit model with acute intracranial hypertension was established by means of inserting balloon inflated with water into epidural space. The rabbits were randomly divided into 3 groups : sham - operation control group, rapid decompression group and controlled decompression group. Observation of the changes of ICP,CT,MRI and the expressions of IL- 1β, Nfκbp65, Bax, Caspase - 3 and apoptosised of nerve cells were performed. Results 24 h after decompression, the incidence of cerebral infarction in the group with controlled decompression was lower than that in the group with rapid decompression. The expressions of IL - 1β,Nfκbp65 ,Bax and Caspase -3 in the group with controlled decompression was lower than those in the group with rapid decompression. The rate of apoptosis in the group with controlled decompression was lower than that in the group with rapid decompression. Conclusions Controlled decompression could lighten cerebral ischemia reperfusion injury and reduce the incidence of cerebral infarction.
