摘 要:目的探讨早期干预治疗及游泳训练对高危儿预后的影响。方法回顾性分析2005年1月—2008年1月我院新生儿重症监护病房(NICU)住院的452例高危儿的临床资料。分为3组:常规育儿组(A组)、干预治疗组(B组)、干预治疗+游泳训练组(C组)。结果 3组新生儿神经行为测定(NBNA)评分比较差异无统计学意义(P>0.05)。12月龄时B组、C组大运动、精细运动、适应行为、语言和个人-社会性行为5大功能区评分较A组明显升高,C组大运动评分较B组明显升高,差异均有统计学意义(P<0.05)。10、12、24月龄时B、C组智能发育指数(MD I)评分较A组明显升高,12、24月龄时C组较B组明显升高,差异均有统计学意义(P<0.05)。6、10、12、24月龄时B、C组心理运动发育指数(PD I)评分较A组明显升高,12、24月龄时C组较B组明显升高,差异均有统计学意义(P<0.05)。3组后遗症发生率比较差异无统计学意义(χ2=1.15,P>0.05)。结论早期干预治疗可以显著提高高危儿发育商数(DQ)、MD I、PD I,如辅以游泳训练则获益更为显著。虽然不能减少后遗症的发生率,但对提高高危儿生活质量具有重要意义,值得临床推广。Objective To explore the effects of early intervention and swimming training on the prognosis of high-risk infants.Methods Data were collected from 452 high-risk infants ever admitted to the NICU of our hospital from January 2005 to January 2008.These patients were divided into three groups:Group A(normal control group),Group B(early intervention group) and Group C(early intervention/swimming training group).Results No significant differences existed among NBNA scores of the three groups(P〈0.05).Scores for coarse movements,fine movements,adaptive behaviors,language,and individual-social behaviors among group B and group C were remarkably higher than those in group A at 12 months of age,with group C showed higher coarse movement scores than group B(P〈0.05).As for mental development index(MDI),group B and C showed higher scores than group A at 10 months,12 months,and 24 months of age,with scores in group C higher than in group B at 12 months and 24 months of age(P〈0.05).As for psychomotor development index(PDI),group B and C showed higher scores than group A at 6,10,12,and 24 months of age,with scores in group C higher than in group B at 12 and 24 months of age(P〈0.05).No significant differences existed in sequela rates among the three groups(χ2=1.15,P〈0.05).Conclusion Early intervention exhibits positive effects on DQ,MDI and PDI among high-risk infants,which is strengthened by the combination of swimming training.Despite the unaffected sequela rate,this treating approach still improves the life quality of the patients.
