异位妊娠药物保守治疗46例临床分析

摘　　要：目的探讨米非司酮、甲氨蝶呤、金刚藤胶囊保守治疗异位妊娠的临床疗效。方法对山西省汾阳医院妇科2007-10-1至2010-10-1住院的46例米用非司酮、甲氨蝶呤和金刚藤胶囊保守治疗的异位妊娠患者进行回顾性分析。结果 46例异位妊娠药物保守治疗有11例失败,治愈率76.09%。血HCG-β≥2000mIU/ml组保守治疗成功率为75%(18/24),血HCG-β<2000mIU/ml组保守治疗成功率77.27%(12/22),两组比较χ2=7.621,P=0.06;无包块异位妊娠患者保守成功率92%(23/25),有包块的异位妊娠患者成功57.14%(17/22),两组比较χ2=0.33P=0.857;保守组住院天数为(11.08±4.52)d,手术组住院天数为(7.81±0.75)d,两组比较t=4.1,P=0.000;保守组治疗费用为(1757.4±278.66)元,手术组治疗费用为(4579.2±982.02)元,两组比较t=15.549,P=0.000。结论异位妊娠保守治疗成功与否与血HCG-β水平关系不大,与有无包块有关,无包块异位妊娠药物保守治疗成功率明显高于有包块异位妊娠者;药物保守治疗需住院时间长但所需费用低。Objective To study the clinical efficacy of conservative treatment of Mifepristone, Methotrexate, Diamond Capsule for ectopic pregnancy. Methods 46 patients with conservative treatment of ectopic pregnancy with mifepristone rice, methotrexate, and Diamond Capsule in Fenyang Hospital of Shanxi Province were retrospectively analyzed. Results Among 46 cases of ectopic pregnancy with conservative treatment, 11 cases failed. The cure rate was 76. 09%. The success rate of Blood HCG-β≥ 2000 mIU/ml of conservative treatment group was 75% （18/24）, and the success rate of blood HCG-β〈 2000 mIU/ml of conservative treatment group was 77.27% （17/22）. The results of the two groups were χ2 = 7. 621 ,P = 0. 06, conservative success rate of ectopic pregnancy without blobwere 92% （23/25）, and conservative success rate of ectopic pregnancy patients with was 57. 14% （17/22） The results of the two groups wereχ2 = 0. 33,P =0. 857; days in hospital of conservative group was （ 11.08 ±4. 52） days, and days in hospital for the surgery group was （7.81 ± 0.75 ） days. The results of the two groups was t = 4. 1 ,P = 0. 000 ; conservative group treatment cost （ 1757.4 ± 278.66） yuan, and the surgery group costs （4579. 2 ±982. 02） yuan. The results of the two groups were t = 15. 549, P = 0. 000. Conclusion The success of conservative treatment of ectopic pregnancy and blood levels of HCG-β have little relationship, but it is related to the existence of the the blob conservative treatment of ectopic pregnancy rate without blob was significantly higher than that with blob of ectopic pregnancy and it will take a long time in hospital for the medical conservative treatment but the treatment cost is low.
