甲状腺功能亢进手术中纤维蛋白胶的应用与观察

摘　　要：目的总结甲状腺功能亢进(甲亢)手术使用纤维蛋白胶不放置引流的临床经验。方法对392例甲状腺Ⅲ度及以下肿大的甲亢手术创面喷洒纤维蛋白胶不放置引流(下称蛋白胶组)与386例甲亢手术切口放置胶管引流(下称引流组)进行比较。结果蛋白胶组切口术后无出血,无切口积液,无切口感染,切口甲级愈合,无不良反应,术后平均住院(4.6±0.5)d;引流组切口放置引流管24～48h,切口引流量平均125ml,切口出血3例(0.77%),切口积液7例(1.81%),切口感染1例(0.26%),术后平均住院(5.8±0.7)d。结论在甲状腺Ⅲ度及以下肿大的甲亢手术中创面喷洒蛋白胶不放置引流是可行的。Objective To summarize the clinical experience of application of fibrin sealant operation for patients with hyperthyroidism instead of postoperatire drainage. Methods 778 cases with hyperthyroidism were divided into fibrin sealant group and drainage group. 392 cases whose thyroid gland were no more than three degree in FG group, received FG spraying onto thyroid operation surface instead of drainage,386 cases in drainage group received drainage. And then analyzed the incision complications and hospitalization time between two groups. Results All cases in FG group had at tained first intention healing without such complications as incision hemorrhage, incisionhydrocele, incision infection or other side effects. The average hospitalization time in FG group was （4. 6 ± 0. 5 ） days. In drainage group, the drainage tube stayed for 24 to 48 hours after thyroid operation and the average drainagevolume was 125 ml. Complications occurred in drainage group were as follows： incision hemorrhage 3 cases （ 0.77% ）, incision hydrocele 7 cases （ 1.81% ）, incisian infection 1 case （ 0. 26% ）. The average hospitalization time in drainage group was （5.8 ±0. 7 ） days. Conclusion It＇ s a feasible, effective and safe method to apply FG spraying instead of drainage in thyroid operation for patients with hy- perthyroidism,whose thyroid gland are no more than three degree as long as the thyroid blood vessels are ligated or sutured effectively.
