利福平注射液-胶囊序贯治疗复治涂阳肺结核强化期的疗效观察

摘　　要：目的观察和分析利福平注射液-胶囊序贯治疗复治阳肺结核强化期的疗效。方法 90例复治涂阳肺结核患者随机分为观察组(45例)与对照组(45例),强化期分别采用HZES+利福平注射液-胶囊序贯治疗和HZES+单纯口服利福平胶囊方案治疗:追踪观察强化期1、2个月末痰菌阴转、肺部病灶吸收及不良反应情况。结果 1、2个月末痰菌阴转率,观察组分别为82.2%、97.8%。对照组分别为62.2%、84.4%;1、2个月末胸部X线影像学检查显效率,观察组分别为17.7%、44.4%,对照组分别为11.1%、26.6%;治疗组与对照组2个月末空洞闭合、缩小率分别为42.2%、22.2%,肝功能异常发生率分别为22.5%、25.0%,胃肠道反应发生率分别为20.0%、45.0%。结论利福平注射液-胶囊序贯治疗起效快、效果佳、不良反应少,在控制症状、缩短排菌时间、减少不良反应及抢救重症结核病上有很好的效果。Objective Observe and analyze the effect of recurrent treated smear-positive pulmonary tuberculosis by rifampin injection-capsule sequential therapy in intensive periods.Methods 90 cases recurrent treated smear-positive pulmonary tuberculosis were separated into two groups randomly：observation group（45 cases） and control group（45 cases）.The two treating schemes：HZES adding rifampin injection-capsule sequential therapy and HZES adding rifampin capsule orally were adopted in intensive periods.Traced conditions of sputum conversion rates and pulmonary lesions absorption and toxic side-effect in the first and second intensive month.Results Sputum conversion rates of observation group and control group in the end of the first and the second month are 82.2%,97.8% and 62.2%,84.4%.Significant efficiency of Chest X-ray imaging examination in the same periods are 17.7%,44.4% and 11.1%,26.6% respectively.Cavity closed and decreasing rates of the two groups are 42.2% and 22.2% at the end of the second month.Rates on dysfunction of livers are 22.5% and 25.0%,and gastrointestinal reaction rates are 20.0%、45.0%.Conclusions Rifampin injection-capsule sequential therapy enjoys the advantages of rapid action,better result and less side-effect.
