泌尿系感染病原菌分布及耐药性分析

摘  要:目的了解泌尿系感染病原菌的分布及对常用抗生素的耐药性进行分析,为临床合理用药提供参考。方法对2010年1~12月送检的622份患者尿液标本分离出297株阳性标本进行病原学和药物敏感性分析。采用WHONET5.0版软件统计分析数据。结果分离的细菌297株病原菌主要为大肠埃希氏菌,其次为肠球菌,再次为真菌。检出大肠埃希氏菌163株,占总数的54.88%,ESBLs阳性菌株86株,阳性率为52.76%;对亚胺培南、呋喃妥因、头孢哌酮/舒巴坦、哌拉西林/他唑巴坦、阿米卡星的敏感性很高,在98.77%~79.75%之间;对头孢菌素、喹诺酮类抗生素的敏感性在73.01%~39.88%之间;对哌拉西林和氨苄西林的敏感性低于20%。结论患者泌尿系感染大肠埃希氏菌对常用抗生素耐药性比较严重,临床应根据药敏试验结果合理使用抗生素,对减少耐药菌株的产生及传播及控制医院感染具有重要意义。Objective To investigate distribution and resistance to commonly used antibiotics of pathogenic bacteria in urinary tract infections in our hospital want to provide the basis for rational clinical application of antibiotics.Methods Retrospectively analyzed on aetiology and drug sensitivity for 297 positive urine sample isolated from 626 patient from Jan to Dec 2010.WHONET 5.0 statstic software was used to analyze the data.Results Totally 297 pathogenic strains,163 E.coli accounted for 54.88% and ranking the top,the followed was Enterococcus spp,the third was fungi.Among 163 E.coli isolates strains 86 extended spectrum β-lactamases(ESBLs)-producing strains were detected and positive rate was 52.76%.The most(98.77%~79.75%)of E.coli were highly susceptible to imipenem,nitrofurantoin,cefoperazone-sulbactam,piperacillin-tazobactam and amikacin.The drug susceptibility rate to cephalosporin,quinolone and gentamicin were 73.01%~39.88%,and the rate to piperacillin and ampicillin were below 20%.Conclusion The resistance of E.coli of urinary tract infection to commonly used antibiotics is high in our hospital.It is significance to reduce the generation and dissemination of resistant strains and to control of hospital infection by rational use of antibiotics.
