临床药师在心血管内科开展药学监护切入点探讨

摘　　要：目的:探讨临床药师在心血管内科开展临床药学服务的切入点。方法:结合临床药师深入心血管内科的工作实践,总结开展药学服务的内容、方法和体会。结果:临床药师运用临床思维及时合理选用药物、掌握适应证及禁忌证、观察药品不良反应、特别对抗凝血药和抗血小板药实施药学监护、注意药物相互作用和强化用药教育作为心血管专业临床药师工作的切入点。结论:临床药师可为临床提供多种形式的药学服务,以使药物应用更加安全、有效。OBJECTIVE： To explore approaches for clinical pharmacists to provide clinical pharmaceutical care in department of cardiology. METHODS： We summarized the content, method and experiences of carrying out pharmaceutical care based on our clinical practice in department of cardiology. RESULTS： Clinical pharmacists in cardiology area provided pharmaceutical care through choosing drugs rationally, mastering indications and contraindication, observing adverse drug reactions, monitoring anticoagulant and antiplatelet drug, paying attention to drug interaction and strengthening medication education. CONCLUSIONS： Clinical nharmacists can provide various nharmaceutical care to achieve safe and effective drue use.
