慢性乙型肝炎病毒携带者外周血T细胞亚群的变化与肝组织病变程度的关系

摘　　要：目的分析乙型肝炎病毒(HBV)携带者T细胞亚群的变化及其与肝组织病变程度的关系。方法选取HBV携带者60例进行肝组织病理检查,应用流式细胞仪检测外周血T细胞亚群的表型和频率,并与16例健康对照者进行比较分析。结果 (1)非活动性HBsAg携带者与对照者比较,外周血CD4+百分率和CD4+/CD8+比值下降,CD8+百分率升高,差异有统计学意义(P<0.05)。慢性HBV携带者与对照者比较,CD4+百分率、CD4+/CD8+比值明显下降,CD8+百分率明显升高,差异有统计学意义(P<0.01)。(2)非活动性HBsAg携带者与慢性HBV携带者CD4+、CD8+百分率与CD4+/CD8+比值比较,差异无统计学意义(P>0.05)。(3)慢性HBV携带者CD4+百分率在G3～4亚组明显高于G0～两亚组(P<0.05),非活动性HBsAg携带者两亚组间比较差异无统计学意义(P>0.05)。结论 HBV携带者体内存在T淋巴细胞亚群失衡和细胞免疫功能紊乱,这可能导致HBV感染后慢性HBV携带。Objective To evaluate the correlation between the peripheral blood T lymphocyte subsets and hepatic pathological changes in HBV carriers. Methods Histopathological examination and flow eytometry were performed in 60 asymptomatic HBV carriers and 12 healthy controls for assessmenl of hepatic pathologic, change and peripheral blood T lymphocyte subsets, respectively. Results （ 1 ） The CD4 ＋ cell percentages and CD4 ＋/CD8 ＋ ratios were significantly reduced, while CD8 ＋ cell percentages were significant increased, in non - active HBsAg carriers and chronic HBV carriers, compared with those in controls （ P 〈 0. 05 ）. （ 2 ） No significant difference in CD4 ＋ or CD8 ＋ cell percentage, or CIM ＋ / CD8 ＋ ratio was observed between non - active HBsAg carriers and chronic HBsAg carriers （ P 〉 0. 05 ）. （ 3 ） Although significant higher CD4 ＋ cell percentage was revealed in G3..4 subgroup than that in G0_2 subgroup in chronic HBV carriers （ P 〈 0. 05 ）, no significant difference was revealed in non - active HBV carriers （ P 〉 0. 05 ）. Conclusion The peripheral blood T -lymphocyte subsets and cellular immunity disorder revealed in asymptomatie HBV carriers may contribute to HBV persistent infection.
