两种术式对非侵袭性上颌窦炎的疗效对比

摘　　要：目的对比鼻内镜下上颌窦开放术及鼻内镜下上颌窦开放+下鼻道开窗术治疗非侵袭性上颌窦炎的效果。方法对2001年1月～2009年8月本院39例非侵袭性真菌性鼻-鼻窦炎的患者,按不同术式进行分组,E组18例,采用鼻内镜下上颌窦开放术;(E+W)组21例,采用鼻内镜下上颌窦开放术联合下鼻道开窗术,术后常规换药、1%H202冲洗。结果随访6个月～4年,E组治愈12例,好转2例,复发4例;(E+W)组治愈17例,好转4例,无复发患者。其中E组复发者经再次经鼻内镜手术+下鼻道开窗术清除病变组织,反复换药及冲洗术腔后治愈。结论两种术式均具有较高的治愈率,但鼻内镜下上颌窦开放+下鼻道开窗术能更好的暴露术腔,有利于完全清除病变组织,避免复发。Objective To compare the response of endoscopic maxilly sinus surgery and endoscopic maxillary sinus surgery plus surgery of Windowing of Inferior nasal meatus method.Methods According to different surgery kinds,39 patients admitted in our hospital between 2001.1 and 2009.8 were divided into two groups.Group E includes 18 patients who received endoscopic maxillary sinus surgery Group（E＋W） includes 21 patients who received endoscopic maxillary sinus surgery plus surgery of Windowing of Inferior Nasal Meatus.All patients were change dressings with nasal endoscope and rinsed with 1%H202 after surgery.Results During a follow up of 6 months to 3 years,12 patients of group E healed,2 became better,4 recrudesced,17 patients of group（E＋W）healed,4 became better,none recrudesced.There were obvious differences between the two groups（P 0.05）,the 4 recrudesced in group E received.surgery of group（E＋W）and The treatment after surgery,then healed.Conclusion Two groups have a high healing rate,but endoscopic maxillary sinus surgery plus surgery of Windowing of Inferior Nasal Meatus can expose the operative region better,it＇s good for removing the abnormal tissues completely and avoiding recurrence.
