GlideScope Ranger视频喉镜在野战急救气管插管中的应用

摘　　要：目的:评价GlideScope Ranger视频喉镜在野战急救困难气管插管中的应用价值。方法:67例需要经口气管插管进行救治的野外伤员,运用GlideScopeRanger视频喉镜(G组)29例,Macingosh直接喉镜(M组)27例,其他11例经直接喉镜操作2次失败改视频喉镜操作,分析比较2组伤员气管插管操作所需时间、插管次数、插管成功率以及插管所致的即刻并发症。结果:2组伤员的一般情况比较差异无统计学意义。G组困难插管一次插管成功率显著高于M组(P<0.05),G组插管时间明显短于M组,而M组一次插管时间短于G组(P<0.05),视频喉镜下插管后即刻并发症显著低于直接喉镜(P<0.05)。结论:野战急救困难气管插管采用GlideScope Ranger视频喉镜插管具有成功率高、速度快、并发症少、体积小巧、方便携带等优点,是提高野战救护质量有效的方法之一。Objective To evaluate the application value of GlideScope Ranger Video laryngoscope in field first aid of difficult tracheal intubation. Methods 67 patients who required intubation in field were analyzed and compared, 29 cases of whom used GlideScope Ranger video laryngoscope （G group） and 27 cases used Macingosh direct laryngoscopy（M group）, another 11 cases among M group, who failed twice in direct laryngoscopy, change into the video laryngoscope operations. We compared the intubation time, intubation times, success rate and the immediate complications. Results Comparison of two groups of the general situation of the wounded was no significant difference. G group of difficult intubation success rate of intubation was significantly higher than M group（P0.05）. Intubation time of G group was shorter than M group, while the intubation time of M group was shorter than G group（P0.05）. immediate complications of Video Laryngoscope intubation was significantly lower than direct laryngoscopy intubation（P0.05）. Conclusion GlideScope Ranger video laryngoscope intubation has advantages in improving the quality of field first aid such as high success rate, high speed, fewer complications, compact, easy to carry, etc.
