^(131)I治疗后甲减患者血清降钙素及降钙素储备功能变化

摘　　要：目的研究经131I治疗后出现甲减患者血清降钙素水平以及降钙素储备功能的变化。方法选取在我科就诊的年龄为21~40岁经131I治疗后并发甲减的甲亢患者及部分经131I治疗的甲状腺非髓样癌患者,分别按性别、甲减发生时间分组。以21~40岁正常人为对照组,分别测定各组血清降钙素水平及降钙素储备功能。结果女性早发甲减组CT基础值为(10.78±3.00)mmol/L,5min降钙素峰值为(17.84±4.94)mmol/L,升高幅度为(7.06±4.14)mmol/L,与对照组差异有统计学意义(P值均为0.00)。女性迟发甲减组CT基础值为(11.28±3.10)mmol/L,降钙素峰值为(17.82±4.02)mmol/L,升高幅度为(6.01±4.07)mmol/L,与对照组差异有统计学意义(P值均为0.00)。女性早发组与迟发组差异无统计学意义(P值分别为0.646、0.879、0.438)。甲癌组降钙素基础值、峰值、升高幅度与对照组差异有统计学意义(P值均为0.00)。男性甲减组CT基础值为(11.98±3.93)mmol/L,降钙素峰值为(17.68±3.42)mmol/L,升高幅度为(5.50±3.33)mmol/L,与对照组差异有统计学意义(P值均为0.00)。结论 131I治疗后甲减患者血清降钙素水平及降钙素储备功能均降低。Objective To study the changes of the serum calcitonin level and calcitonin reserve capacity in patients with hypothyroidism after 131↑I therapy.Methods The subjects singled out in my treatment were divided into groups by both gender and time of hypothyroidism-occurrence.These patients,at the age of 21-40 years,were with hyperthyroidism secondary hypothyroidism or thyroid cancer after the treatment by 131↑I.Some health person at the age of 21-40 years were taken as the control group.Then the serum calcitonin levels and calcitonin reserve function of each group were measured at variational times.Results In the group of women with early-onset hypothyroidism the CT basal value was（10.78±3.00） mmol/L,the mean value of peak calcitonin was（17.84± 4.94） mmol/L,the mean value of the creased range was（7.06± 4.14） mmol/L,were with significant difference compared to the control group（P=0.00,0.00,0.00） and with no significant difference compared to the late-onset group.In the group of women with late-onset hypothyroidism the CT basal value,the mean value of peak calcitonin and the mean value of the creased range were with significant difference compared to the control group（P=0.00,0.00,0.00）.In the thyroid cancer group,there was significant difference compared to the control group in the CT basal value,the mean value of peak calcitonin and the mean creased range（P=0.00,0.00,0.00）.In the group of men with hypothyroidism the CT basal value was（11.98±3.93） mmol/L,the mean value of peak calcitonin was（17.68±3.42） mmol/L,the mean value of the increased range was（5.50±3.33） mmol/L,were with significant difference compared to the control group（P=0.00）.Conclusion The serum calcitonin levels and calcitonin reserve function were decreased in patients of hypothyroidism after 131↑I therapy.
