摘　　要：目的探讨无创正压通气治疗慢性阻塞性肺疾病(COPD)Ⅱ型呼吸衰竭急性期的临床效果和护理方法。方法对本院COPD合并Ⅱ型呼吸衰竭42例患者使用无创性呼吸机治疗并给予相应的护理,比较患者治疗前后临床症状和血气分析的变化。结果经治疗患者呼吸困难的症状明显减轻,心率恢复正常,血气分析动脉学氧分压和二氧化碳分压明显改善,治疗前后以上生理指标间差异有统计学意义(P<0.05)。结论无创呼吸机治疗COPD合并Ⅱ型呼吸衰竭可避免气管切开或插管,临床疗效显著,细心的护理可有效地提高患者的生活质量,促进患者恢复。Objective To explore the efficacy and nursing care of applying noninvasive positive pressure ventilation in the treatment of chronic obstructive pulmonary disease（COPD） associated with type Ⅱ respiratory failure.Methods 42 patients of COPD associated with type Ⅱ respiratory failure were treated with noninvasive positive pressure ventilation and given corresponding nursing.The clinical symptoms and blood gas analysis before and after treatment were compared.Results After the treatment,symptoms of breathing difficulties reduced significantly,and heart rate returned to normal.Arterial blood gas analysis showed that the partial pressure of oxygen and carbon dioxide improved obviously after the treatment.There was statistically significance in between the data before and after the treatments（P 〈0.05）.All the patients received effective psychological care,useful guidance for treatment,active measures for prevention of complications,as well as the strengthening of clinical testing,which improves their quality of life.Conclusion In the treatment of COPD associated with type Ⅱ respiratory failure,noninvasive positive pressure ventilation is simply operated and has small trauma.It avoids the risk of tracheal intubation or tracheostomy and decreases the incidence of complication.Nursing care improves the life quality of patients effectively,and facilitates the recovery of the disease.
