动力髋螺钉钢板系统和股骨近端锁定钢板治疗股骨粗隆间骨折的疗效观察

摘　　要：目的观察动力髋螺钉钢板系统和股骨近端锁定钢板治疗股骨粗隆间骨折的疗效。方法回顾性分析2006年8月～2011年1月我院用动力髋螺钉钢板系统和锁定钢板治疗股骨粗隆间骨折各47例和28例的疗效。结果骨折处均顺利愈合,无切口感染。因骨质疏松骨折碎裂髋内翻2例。Harris评分系统:动力髋螺钉优良率76.6%(36例),锁定钢板优良率89%(25例),统计学无差异。结论动力髋螺钉钢板系统和股骨近端锁定钢板治疗股骨粗隆间骨折,疗效肯定,手术后病人早期行功能锻炼,可有效预防长期卧床的并发症。锁定钢板内固定较动力髋螺钉更利于病人早期活动。Aim To observe the curative effect of dynamic hip screw（DHS）plate system and proximal femoral locking plate in treatment of intertrochanteric fractures.Methods Retrospective analysis curative effect of patients in our hospital from August 2006 to January 2011,47 cases of patients were treated with dynamic hip screw plate system and 28 cases of patients with proximal femoral locking plate.Results Healing of fractures was successfully.Fracture healed with no infections.Only 2 patients with significant comminution and bone loss,had coxa adducta.Harris scoring system：An excellent or good functional outcome was achieved in 36 patients（76%）in DHS group and 25 patients（89%）in locking plate group.There were no statistical differences with respect to the ratio of excellent and good functional.Conclusions DHS and proximal femoral locking plate in the treatment of intertrochanteric fracture were more effective,functional exercise in early stage after surgery can effectively prevent the complications of long-term bed rest.Locking plate fixation is more conducive than the dynamic hip screw patients in early mobilization.
