摘  要:目的比较咪达唑仑-氯胺酮静脉麻醉复合骶管阻滞在小儿腹股沟疝手术中的麻醉效果。方法选择80例腹股沟疝手术患儿,分为复合骶管阻滞组(1组)和单纯静脉麻醉组(2组)。1组咪哒唑仑-氯胺酮静脉麻醉后行骶管阻滞,2组单纯咪哒唑仑-氯胺酮静脉麻醉,术中视情况间断推注氯胺酮维持麻醉效果。分别记录两组在麻醉前(T0)、手术后20分钟(T1)及术毕时平均动脉压(MAP)、血氧饱和度(SpO2)、心率(HR)、术中氯胺酮用量、术后苏醒时间、术后麻醉不良反应等。结果两组SpO2、HR、MAP差异无统计学意义(P>0.05),1组术中氯胺酮用量明显少于2组(P<0.05),术后苏醒时间明显短于2组(P<0.05),术后麻醉不良反应差异无统计学意义(P>0.05)。结论 咪达唑仑-氯胺酮静脉麻醉复合骶管阻滞应用于小儿腹股沟疝手术具有显著的麻醉效果。Objective To evaluate the effect of Midazolam plus Ketamine by intravenous anesthesia and caudal block in inguinal hernia surgery in children.Methods Eighty cases of inguinal hernia were randomly divided into two groups.Group 1 underwent sacral block after intravenous anesthesia,group 2 underwent intravenous anesthesia only.Anesthesia was maintained by intermittent bolus injection of Ketamine during the surgery appropriately.The mean arterial blood pressure(MAP),oxygen saturation(SpO_2),heart rate(HR),intraoperative dosage of Ketamine and postoperative recovery time,postoperative adverse reactions were recorded before anesthesia and 20 minutes after surgery.Results SpO_2,HR,MAP between the two groups were not statistically different(P0.05).The intraoperative dosage of Ketamine in group 1 was significantly less than that in group 2(P0.05).The postoperative recovery time in group 1 was significantly shorter than that in group 2(P0.05).The postoperative adverse reaction reported no significant difference between the 2 growps(P0.05).Conclusion Midazolam plus Ketamine by intravenous anesthesia combined with caudal block is safe and effective in inguinal hernia surgery in children.
