无夹3孔法腹腔镜阑尾切除术580例临床分析

摘　　要：目的探讨腹腔镜阑尾切除术中无夹电凝处理阑尾系膜及普通丝线结扎阑尾根部的可靠性、优越性及安全性。方法回顾分析2004年5月至2010年5月间无夹3孔法腹腔镜阑尾切除术580例患者的临床资料。结果手术顺利完成576例,中转开腹4例(占0.7%);其中包括肥胖患者56例;异位阑尾22例;6～12岁儿童患者同时合并斜疝11例;手术时间15～150min,术中出血0～25ml,平均住院日4.5d,随访2个月至5年半,术后无肠系膜出血、阑尾残端瘘、肠粘连等并发症发生。结论无夹3孔法腹腔镜阑尾切除其方法安全、可靠可行。Objective To investigate the reliability,advantage and safety of laparoscopic appendectomy for mesoappendix by electrocoagulation and ligation of the root of the appendix with suture silk.Methods Clinical data of 580 patients treated with laparoscopic appendectomy without using clips between May 2004 and May 2010 were analyzed retrospectively.Results 576 patients underwent successfully laparoscopic appendectomy,4（0.7%） received intraoperative conversion to open laparotomy.There were 56 obese patients,22 patients with ectopic appendix and 11 patients with gangrenous appendix root.There were 11 children of 6-12 years old simultaneously sufferred indirect hernia.Operation time was 15-150 minutes.Blood loss was 0-25ml.The average hospital stay of the patients was 4.5 days.580 patients were followed up for 2 months to 5.5 years.Conclusions Laparoscopic appendectomy without using clips is safe and reliable.
