2型糖尿病合并脑梗死患者血清VEGF、ES水平及V/E比值的临床研究

摘　　要：目的:测定2型糖尿病合并脑梗死患者血清血管内皮生长因子VEGF(vascular endothelialgrowth factor)、内皮抑素ES(endostatin)水平,计算V/E比值,探讨3者的变化及临床意义。方法:采用双抗体夹心(ELISA)法测定3组在不同时间点血清VEGF、ES水平。结果:2型糖尿病合并脑梗死组(DM+CI组)、单纯脑梗死组(CI组)、2型糖尿病无并发症组(DM组)血清VEGF水平均高于对照组(P<0.05或P<0.01),CI组(各梗死面积组)均高于DM组(P<0.05或P<0.01),DM+CI组与DM组VEGF水平无显著性差异(P>0.05);ES在CI各组中发病1d并无明显升高,7d浓度显著升高,14d时降低;ES水平在DM+CI组各时间点差异不明显(P>0.05);CI组在发病1dV/E比值高于对照组(P<0.05),7d、14d下降;CI组中大面积梗死组V/E值最高。结论:2型糖尿病合并脑梗死患者的VEGF、ES水平、V/E比值在脑梗死后未出现动态变化,这也可能是2型糖尿病合并脑梗死患者病情重、预后差的原因之一。Objective： To determine the the levels of the vascular endothelial growth factor（VEGF）、Endostatin（ES） and calculate the ratio of V/E in type-Ⅱ diabetes patients with cerebral infarction,then to discuss their changes and clinical significance.Methods： level of VEGF in serum and level of ES were determined by ELISA in type-II diabetes patients with cerebral infarction（DM＋CI group）,cerebral infarction patients（CI group）,patients with only type II diabetes（DM group） and healthy persons（control group） at different time.Results： Level of VEGF in serum in DM＋CI group,CI group and CI group were significantly higher than that of the control group（P0.05 or P0.01）,CI group was higher than DM group（P0.05 or P0.01）,and there were no obvious difference between the DM＋CI group and the DM group（P0.05）.ES level in all CI groups did not have any obvious changes on the 1st day of onset but significantly rose on the 7th day and slight declined on 14th day.There was no difference with the ES level in DM＋CI group on different time point（P0.05）.The ratio of V/E in CI group was higher than that of the control group on the 1st day of onset（P0.05）,then declined on the 7th and 14th day,but which reached the highest in large size of CI group.Conclusions： The level of VEGF,ES and the ratio of V/E in type-Ⅱ diabetes patients with cerebral infarction don＇t have any dynamic variation after cerebral infarction,which may indicate one of the reasons of the worsening of the illness and poor prognosis.
