“产前-产时-产后”一贯制服务模式在非高危孕妇管理中的应用

摘　　要：目的:探讨"产前-产时-产后"一贯制服务模式在非高危孕产妇围产期产科服务中的作用。方法:通过建立"产前-产时-产后"一贯制的管理运行模式,以"组"为单元,为在同济大学附属第一妇婴保健院进行产检和分娩的非高危孕产妇提供产科服务,评价其对降低剖宫产率、降低医疗费用和提高孕产妇满意度的作用。结果:研究组的剖宫产率显著低于对照组(P<0.05);引产或产程中分别有24.1%和30.7%的产妇提出剖宫产的要求,研究组的孕产妇更加易于接受产科医师的建议而继续试产(P<0.05);产科服务模式能降低阴道分娩(P<0.05)和剖宫产费用(P<0.05);研究组的孕产妇对产科服务的满意率明显增加。结论:"产前-产时-产后"一贯制的产科服务模式能降低医疗费用和剖宫产率,提高孕产妇满意率。Objective：To explore the role of ＂antenatal-intrapartum-postnatal＂ all-through service mode in obstetrical service of non-high risk pregnant women during perinatal period. Methods：＂Antenatal-intrapartum-postnatal＂ all-through management operation mode was established,and ＂group＂ was designed as a unit,then obstetrical service was supplied to the non-high risk pregnant women who received prenatal examination and deliveried in the hospital,its effect on reducing the rate of cesarean section and medical expenses and increasing the satisfaction of pregnant women were evaluated. Results：The rate of cesarean section in study group was significantly lower than that in control group（P0.05）.24.1% of the pregnant women in study group put forward cesarean section during induced labor or delivery,while in control group,the percentage was 30.7%,the pregnant women in study group were prone to accepting the advise of obstetricians and continuing trial of labor（P0.05）;the obstetrical service mode could reduce the expenses of vaginal delivery and cesarean section（P0.05）;the satisfaction of pregnant women in study group to obstetrical service increased significantly. Conclusion：＂Antenatal-intrapartum-postnatal＂ all-through obstetrical service mode can reduce medical expenses and the rate of cesarean section,increase the satisfaction of pregnant women.
