摘 要:目的:探讨妊娠期糖尿病(GDM)患者腹部皮下和网膜脂肪组织Omentin基因表达的差异及其与糖代谢水平的相关性。方法:将43例GDM孕妇与43例正常孕妇(NGT)按年龄、产次、分娩前BMI、分娩孕周配对后,测量两组产前空腹血脂水平,并于剖宫产术中留取研究对象腹部皮下和网膜脂肪组织,应用实时定量-聚合酶链反应检测不同部位脂肪组织OmentinmRNA表达水平,分析Omentin mRNA表达水平与OGTT结果和产前脂代谢的相关性。结果:①在GDM人群中,皮下及网膜脂肪组织的Omentin mRNA的表达水平均显著低于NGT组对应部位的表达水平。②在单因素分析中,网膜脂肪组织的Omentin mR-NA表达量与OGTT空腹血糖和餐后2 h血糖呈显著负相关;而腹部皮下脂肪组织的Omentin mRNA表达量则与OGTT餐后2 h血糖水平以及产前HDL-C水平呈显著相关性。③进一步进行多元逐步回归分析显示,网膜脂肪组织和皮下脂肪组织Omentin mR-NA表达量均是OGTT餐后2 h血糖水平独立预测因子,网膜脂肪组织Omentin mRNA表达量对OGTT餐后2 h血糖水平影响更大。结论:网膜脂肪组织Omentin mRNA表达与糖代谢的关系可能比皮下脂肪组织更为密切。Objective:To explore the difference of omentin mRNA expression in subcutaneous and omental adipose tissues and its correlation with glucose metabolism in the patients with gestational diabetes mellitus(GDM). Methods:43 pregnant women with GDM and 43 normal pregnant women were paired according to age,parity,BMI before delivery and gestational weeks,the levels of fasting blood lipid before delivery in the two groups were measured,the subcutaneous and omental adipose tissues were obtained during cesarean section,RT-PCR was used to detect the expression levels of omentin mRNA in adipose tissues of different parts,the correlation between the expression level of omentin mRNA and OGTT result,prenatal lipid metabolism was analyzed. Results:The expression levels of omentin mRNA in subcutaneous and omental adipose tissues in GDM group were significantly lower than those in control group.Single factor analysis showed that there was a significant correlation between the expression level of omentin mRNA in omental adipose tissues and fasting blood glucose and 2-hour postprandial blood glucose of OGTT;while there was a significant correlation between the expression level of omentin mRNA in subcutaneous adipose tissues and 2-hour postprandial blood glucose of OGTT and prenatal HDL-C level.Multiple stepwise regression analysis showed the expression levels of omentin mRNA in subcutaneous and omental adipose tissues were independent predictors of 2-hour postprandial blood glucose of OGTT,the effect of omentin mRNA expression level in omental adipose tissues on 2-hour postprandial blood glucose of OGTT was larger. Conclusion:The relationship of glucose metabolism with omentin mRNA expression in omental adipose tissues is closer than its relationship with omentin mRNA expression in subcutaneous adipose tissues.
