脾蒂结扎法在腹腔镜脾切除术中的应用

摘　　要：目的探讨脾蒂结扎法在腹腔镜脾切除术(LS)中的应用价值及临床疗效。方法 2009年1月至2011年3月对8例难治性特发性血小板减少性紫癜和4例遗传性球形红细胞增多症(其中3例合并胆囊结石)患者,采用腹腔镜下超声刀离断脾周韧带,丝线结扎并钛夹夹闭脾蒂,切除脾脏。合并胆囊结石者,同时切除胆囊。结果 12例均完成腹腔镜脾切除术,平均手术时间102 m in,术中出血平均90 ml,无中转开腹,术后2例切口皮下血肿。结论脾蒂结扎法行腹腔镜脾切除术,操作简单,安全可靠,手术费用低,是最易推广应用的手术方法。Objective To evaluate the value and clinical efficacy of ligation of the splenic pedicle for laparoscopic splenectomy（LS）.Methods Eilght cases of refractory idiopathic thrombocytopenic purpura and 4 cases of hereditary spherocytosis（including 3 cases of gallbladder stones） from January 2009 to March 2011 were removed of the spleen using scalpel ligament transection of the spleen and titanium clip ligation and clip thread closed splenic pedicle in the laparoscopic.Gallbladder stone,while removal of the gallbladder.Results The laparoscopic splenectomy was successfully completed in all the 12 cases.The mean operation time was 102 minutes,and the mean estimated intraoperative blood loss was 90 ml.There were no conversion to open surgery,2 cases of incisional subcutaneous hematoma.Conclusion The ligation of the splenic pedicle is a simple,safe,reliable,low cost of surgery and most simple to promote the use of surgical methods for completing laparoscopic splenectomy.
