摘 要:目的:观察在腹膜透析液中添加丹参酮ⅡA对腹膜透析患者腹腔局部及全身氧化应激状态的影响。方法:将38例终末期肾脏病行持续非卧床腹膜透析治疗的患者随机分为实验组和对照组,每组19例。实验组在腹膜透析液中添加丹参酮ⅡA注射液(每1 L腹膜透析液中加入5 mg丹参酮ⅡA注射液),对照组腹膜透析液中不加丹参酮ⅡA注射液。两组均行标准持续非卧床腹膜透析(每次2 L,每日交换4次)治疗,第23天比较两组患者用药前后血清及透出液氧化应激指标[谷胱甘肽(GSH)、总超氧化物岐化酶(T-SOD)、丙二醛(MDA)]以及透析充分性等指标的变化。结果:实验组患者血清及透出液中GSH、T-SOD均较用药前显著升高(P<0.01或P<0.05),MDA显著降低(P<0.05),患者血清白蛋白、总肌酐清除率(Ccr)、尿素氮清除指数(KT/V)、超滤无明显变化。实验组与对照组比较,患者血清及透出液中GSH、T-SOD显著升高(P<0.05),MDA显著降低(P<0.05)。结论:在腹膜透析液中短期应用丹参酮ⅡA可以改善患者腹腔局部及全身的氧化应激状态。Objectlve:To investigate the impact of Tanshinone IT A on oxidative stress in continuous ambulatory peritoneal dialysis (CAPD) patients. Methods:Thirty-eight stable CAPD patients were randomly divided into two groups. The experimental group (n=lg) were dialyzed with peritoneal dialysis fluid along with 5 mg L-1 Tanshinonc lI A, and the control group ( n = 19 ) were dialyzed with peritoneal dialysis fluid without Tanshinone 11 A. The concentration of glutathione ( GSH), total- superoxide dismutase ( T- SOD), malondialdehyde (MDA) in serum and effluent and serum albumin were measured at the beginning and three weeks later. And dialysis adequancy indices such as KT/V and creatinine clearance rate(Ccr) were also calculated for each group. Results: Three weeks later, the serum and effluent GSH, T-SOD increased and MDA decreased significantly in the experimental group as compared with those indices at the beginning ( P 〈 0.01 or P 〈 0.05 ). No such differenceswere found in the control group. The levels of the serum and effluent GSH, T-SOD were significantly higher, and MDA was lower in experimental group as compared with those in the control group after three weeks treatment( P 〈 0.01 or P 〈 0.05). There were no significant differences in serum albumin,KT/V,uhrafihration volume and Ccr between two groups. The levels of GSH, T-SOD were significantly higher and the levels of MDA was significantly lower in experimental group compared with those in control group (P 〈 0.05 ). Conclusion : Shortterm supplement of Tanshinone IIA into peritoneal dialysis fluid can reduce both local and systemic oxidative stress in CAPD patients.
