超声探查对降低PICC并发症的研究

摘　　要：目的了解应用超声探查后经颈外静脉穿刺置入中心静脉导管(PICC)对降低PICC合并症的影响。方法回顾性了解穿刺前行超声检查确定血管形态、走行及周边情况后行颈外静脉PICC的患者409例,常规经验性评估行颈外静脉PICC患者386例,分析两组相关并发症发生情况。结果超声探查组置管成功率明显高与非探查组,静脉炎由6.0%降至1.2%,血栓发生率由10.1%降至2.0%,导管易位率由3.9%降至0.5%,差异有统计学意义(P<0.01)。结论置管前行血管超声确定颈外静脉分型再行置入PICC,可有效地降低PICC合并症。Objective To investigate the efffection of the ultrasonic to reduce the complications of PICC.Methods Retrospective study on 409 cases of patients using the ultrasound measuring vessels inner diameter,ntima,blood flow,arterio and venous distance,and the surrounding lymph nodes and other circumstances before puncture,and 386 cases of patients with PICC,then analysis all the complications of the PICC.Results Two groups of external carotid venous catheter patients compared,the complications of ultrasonic group were lower than the nonultrasonic group,phlebitis by 6.0% drop to 1.2%,blood clots by 10.1% drop to 2.0%,ductal translocation rate by 3.9% drop to 0.5%,this results has statistically significance（P0.01）.Conclusions According to the vascular standards of ultrasound screening PICC for indications which can effectively reduce PICC complications.
