提高交锁髓内钉远端锁钉锁定成功率的方法研究

摘　　要：目的探讨交锁髓内钉技术治疗下肢骨折远端锁钉锁定失败的原因和提高锁定成功率的方法。方法 2004年3月～2009年5月采用国产交锁髓内钉治疗下肢骨折128例,早期23例术中出现远端锁定钉锁定困难,就其出现的原因进行分析,并制定了一套提高锁定成功率的标准手术操作方法和出现锁定困难时的处置方法。结果本组128例病人远端锁定钉皆成功锁定,23例锁定困难的病例皆及时找出原因并处置后成功锁定,锁定成功率达100%。结论行交锁髓内钉内固定手术时,术前要充分准备,术中要按标准手术操作方法进行操作,遇到锁定困难时要善于分析找出原因,并从失败中进行总结,能提高锁定成功率。Objective To discuss the intramedullary nail treatment technique by means of distal locking screw to lock lower limb fracture.Methods From March,2004 to May,2009,domestic interlocking nail was used for the treatment of 128 cases of lower limb fracture.Early distal locking screws were used in 23 cases and difficult locking was found.Analysis was made and a set of surgical procedures for locking were made to solve the problem.Results 128 patients got successful distal locking.Only 23 cases got some difficulty but were successful in the end.Conclusion The interlocking intramedullary nail surgery should be fully prepared preoperatively,and the intraoperative surgical procedures should be carried out according to the standard to improve the locking rate.
