PPH联合外痔分段环切缝合术的临床应用价值

摘　　要：为探讨PPH联合外痔分段环切缝合术的临床应用价值，回顾分析行PPH联合外痔分段环切缝合术治疗的重度混合痔（试验组，83例）和单纯行PPH治疗的Ⅱ、Ⅲ期内痔及以内痔为主的混合痔（对照组，61例）患者资料，并对术后水肿、出血、疼痛、尿潴留、肛门狭窄等并发症及患者满意度进行对比分析。结果显示，两组肛门创口渗血、疼痛、尿潴留、肛门狭窄发生率及患者满意度方面差异无统计学意义，P〉O．05；试验组水肿发生率高于对照组，P〈O．05。结果表明，PPH联合外痔分段环切缝合术治疗重度混合痔能够达到单纯行PPH治疗Ⅱ、Ⅲ期内痔及以内痔为主的混合痔的疗效，且并发症能够控制在理想范围，疗效满意。其在一定程度上拓展了传统PPH的适用范围，可加以推广。In order to explore the clinical applicable value of the procedure of PPH combined with segmental circumferential resection suture for external hemorrhoids, authors analysed the data from the 83 cases （as trial group） who had been subjected to above procedure for treating their severe mixed hemorrhoids, and from the 61 cases （as controlled group） who to alone PPH for treating their II , III stage internal hem- orrhoids and predominated by internal hemorrhoids mixed hemorrhoids, retrospectively, and compared and analysed their postoperative edema, bleeding, pain, uroschesis and anal stenosis, such as complications, as well patient＇s satisfactory degree of both groups. As results, except for edema incidence（ P 〈0.05）, above rest items had all not statistical differences（ P 〉0.05）. Results show that the procedure of PPH combined with segmental circumferential resection suture for external hemorrhoids in the treatment of severe mixed hemorrhoids can get the same therapeutic effect as one got in the treatment of the II , III stage internal hemorrhoids and predominated by internal hemorrhoids mixed hemorrhoids with alone PPH,and postoperative complications could be controlled in ideal range, its therapeutic effect is satisfactory, thus which at certain extent extended and popularized application scale of PPH technique.
