摘  要:目的:探讨全氟化碳(PFC)汽化吸入预处理对油酸型兔急性肺损伤(ALI)早期炎症因子的干预作用。方法:实验兔12只随机分为对照组和PFC预处理组,每组6只。动物麻醉后气管插管行机械通气,对照组注油酸造ALI模型后行机械通气120 min,预处理组在造模前先于经气管内汽化吸入PFC 60 min,再注油酸造ALI模型后行机械通气120 min。两组分别于麻醉通气30 min(基础值)、ALI造模成功时、ALI后30、60、90、120 min各时点检测氧合指数、PaO2、PaCO2。实验结束后留取静脉血离心取上清液,对左肺进行肺灌洗留取肺灌洗液,应用ELISA法检测血清及肺灌洗液中TNF-α、IL-1β的含量。结果:与对照组比较,PFC预处理组在各时点的氧合指数和PaO2均明显升高、PaCO2明显下降(P均<0.05)。PFC预处理组血清和肺灌洗液中TNF-α的含量分别为(211±1)ng/L、(326±9)ng/L,明显低于对照组(226±2)ng/L、(368±11)ng/L(P均<0.05);PFC预处理组血清和肺灌洗液中IL-1β的含量分别为(77±2)ng/L、(93±2)ng/L,也明显低于对照组(99±3)ng/L和(116±2)ng/L(P均<0.05)。结论:经气道汽化吸入PFC预处理60 min,可改善油酸型ALI实验兔的氧合状况,减少早期炎症因子TNF-α、IL-1β的释放。Objective: To assess the intervention effects of intratracheal administration of vaporized perfluorocarbon (PFC) on early stage inflammatory factors in rabbits with oleic acid-induced acute lung injury (ALI). Methods: Twelve New Zealand rabbits were randomly divided into the control group and the PFC pretreatment group (n = 6). All rabbits were anesthetized and mechanically ventilated by tracheal intubation. After ALI was induced by oleic acid (OA), all rabbits were mechanically ventilated for 120 min, while vaporized perfluorocarbon was given intratracheally for 60 min before OA administration in PFC pretreatment group. PaO2/FiO2, PaO2 and PaCO2 were measured at baseline, when ALI was just established, and 30 min, 60 min, 90 min, 120 min after ALI. After obove indicated treatments, samples of the blood serum were obtained by centrifugation of the venous blood, and lung lavage fluid was obtained by lavaging the left lung. The concentrations of TNF-α and IL-1β in blood serum and lung lavage fluid were detected by ELISA. Results: Oxygenation index and PaO: in PFC pretreatment group rised significantly comparing with the control group (P 〈 0. 05 ). PaCO2 decreased significantly in PFC pretreatment group when compared with the control group (P 〈 0. 05 ). The concertration of TNF-α in blood serum and lung lavage fluid in PFC pretreatment group were significantly lower than that in control group ( P 〈 0. 05 ), which were (211 ± 1 ) ng/L and (326 ±9) ng/L vs (226 ±2) ng/L and (368 ± 11 ) ng/L. The concertration ofIL-1 β in blood serum and lung lavage fluid were significantly decreased in PFC pretreatment group ( P 〈 0. 05 ), which were (77 ±2) ng/L and (93 ±2) ng/L vs (99 ±2.7) ng/L and (116 ±2) ng/L. Conclusion: Intratracheally vaporized PFC pretreatment for 60 min can improve oxygenation and reduce the release of TNF-α and IL-1β in rabbits with OA-induced ALI.
