微动敏感床垫式睡眠监测系统对阻塞性睡眠呼吸暂停低通气综合征诊断作用的研究

摘　　要：目的检验微动敏感床垫式睡眠监测系统(micro-movement sensitive mattress sleep monitoring system,MSMSMS)在诊断阻塞性睡眠呼吸暂停低通气综合征(OSAHS)中的准确性,以期为临床提供一种简易、准确的监测方法。方法 54例以睡眠打鼾为主诉的门诊患者同时进行MSMSMS和多道睡眠图(PSG)监测,比较两种监测方法各指标的差异,以检测MSMSMS的准确性。结果与PSG比,MSMSMS对OSAHS诊断的敏感性为98%,特异性为100%。对病情分度判断的符合率分别为轻度57%,中度80%,重度97%。两种方法所得呼吸暂停次数、呼吸暂停指数、阻塞性次数、阻塞性指数、混合性次数、混合性指数、中枢性次数、中枢性指数等均无显著性差异。结论 MSMSMS诊断OSAHS具有很高的敏感性,尤其对重度OSAHS患者诊断更为准确,对特定人群是一种简单、可靠的替代方法。OBJECTIVE To test the accuracy of the micro-movement sensitive mattress sleep monitoring system（MSMSMS） in diagnosing the obstructive sleep apnea hypopnea syndrome.METHODS Fifty four outpatients with chief complaint of sleep snoring were recruited to simultaneously undergo a test of MSMSMS and polysomnography（PSG）.Two experienced technicians conducted a double-blind analysis of the two results respectively,and the differences of various data between the two monitoring methods were compared so as to test the sensitivity and specificity of MSMSMS.RESULTS Compared with PSG,the sensitivity and specificity of MSMSMS in diagnosis of OSAHS was 98% and 100% respectively.The consistency of diagnostic grading of the patients＇ severity were 57%（for mild cases）,80%（for moderate cases） and 97%（for severe cases）.In the results of the two methods,there were no obvious differences（P0.05） in apnea time,apnea index,obstructive frequency,obstructive index,mixed apnea time,mixed apnea index,central apnea frequency,central apnea index,etc.CONCLUSIONThe MSMSMS is highly sensitive for diagnosing the sleep apnea syndrome,especially more accurate for the patients with severe sleep apnea patients.The MSMSMS is a simple and reliable alternative method for a certain group of patients.
