实时三维超声心动图与磁共振对法洛四联症患者心室功能的对比研究

摘　　要：目的以磁共振(MRI)检测结果为标准,探讨实时三维超声心动图(RT-3DE)检测法洛四联症(TOF)患者左、右心室功能的准确性。方法 16例TOF患者行常规超声心动图检查并确诊后,分别使用RT-3DE和MRI采集患者的左、右室全容积数据库,分析左、右室舒张末期容量(LVEDV、RVEDV)、收缩末期容量(LVESV,RVESV),计算射血分数(EF),比较两种检测方法得出的RVEDV、RVESV及EF值的相关性和差异。结果 RT-3DE和MRI检测TOF患者RVEDV、RVESV、RVEF、LVEDV、LVESV及LVEF分别为(49.1±22.1)ml、(22.4±11.4)ml、(54.4±10.9)%、(40.9±19.9)ml、(20.1±11.4)ml、(51.42±7.6)%和(50.3±20.2)ml、(22.8±10.8)ml、(54.9±9.9)%、(41.64±19.00)ml、(20.6±10.5)ml、(51.11±6.00)%,r值分别为0.996、0.997、0.950、0.996、0.998和0.950。结论 RT-3DE能方便、准确地检测TOF患者心室容积并计算其收缩功能。Objective MRI detection results as standard, to evaluate ventricular systolic function in patients with tetralogy of Fallot （TOF） by real - time three - dimensional echocardiography （ RT - 3DE）. Methods Sixteen patients with TOF were examined and confirmed by conventional echocardiography. Their ventricular full volume imaging was acquired by RT - 3 DE and MRI, the left ventricular end - diastole volume （ LVEDV ）, right ventricular end - diastole volume （ RVEDV）, left ventricular end- systole volume （LVESV） and right ventricular end -systole volume （RVESV）were analyzed, the ejection fraction （EF）was calculated. Then the correlation and difference of RVEDV, RVESV and EF in left and right ventricle were compared. Results The RVEDV, RVESV, RVEF, LVEDV, LVESV and LVEF in patients with TOF detected by RT - 3DE and MRI were （49.1±22.1）ml、（22.4±11.4）ml、（54.4±10.9）%、（40.9±19.9）ml、（20.1±11.4）ml、（51.42±7.6）%和（50.3±20.2）ml、（22.8±10.8）ml、（54.9±9.9）%、（41.64±19.00）ml、（20.6±10.5）ml、（51.11±6.00）% respectively,r were 0. 996, 0. 997, 0. 950,0. 996, 0. 998 and 0. 950, respectively. Conclusion RT - 3DE can determine the ventricular volume and systolic function conveniently and accurately in patients with TOF.
