肺部卡波济氏肉瘤50例影像表现

摘　　要：目的提高对艾滋病并发肺部卡波济氏肉瘤的影像表现的认识。方法收集50例HIV阳性并发肺部卡波济氏肉瘤的病例,回顾分析其胸部X线和CT影像表现并结合治疗结果进行判断。结果胸部X线45例(90%)显示弥漫性间质浸润;40例(80%)见胸腔积液,其中双侧32例,单侧8例;18例(36%)多发结节影。其中最为典型的影像表现为,弥漫性间质浸润影中间杂多发结节影。胸部CT提示肺部结节影和肺门周围斑片影,弥漫性间质浸润沿气管-血管周围分布或沿肺小叶间裂分布。治疗好转病例斑块缩小;治愈者遗留纤维条索影,斑块显著缩小。结论肺部卡波济氏肉瘤的典型的影像表现为,弥漫性间质浸润影中间杂多发结节影;表现为现其他形式者,需结合治疗结果判定。Objective To enhance the understanding of imaging presentations of AIDS-associated pulmonary Kaposi＇s sarcoma.Methods Fifty cases of AIDS-associated pulmonary Kaposi＇s sarcoma were collected and their X-ray and CT presentations were analyzed retrospectively.Results Chest X-ray： Diffuse interstitial infiltrates（45/50）;pleural effusion（40/50）;multiple nodular shadows（18/50）.Chest CT： better definition of pulmonary nodules and perihilar patchy shadows;diffuse peribronchovascular infiltrates and infiltrates along interlobular fissures.Among one cured case,X-ray presented only residual linear shadows and markedly shunk nodular-patchy shadows.Conclusion The characteristic imaging presentations are diffuse interstitial infiltrates mingled with multiple nodular shadows,if presented in other ways,the diagnosis should be correlated with therapeutic results.
