妊娠期高血压病患者血小板-白细胞聚集体检测的临床意义

摘　　要：目的:探讨妊娠期高血压病(HDCP)患者血小板-白细胞聚集体(PLA)表达百分率变化及临床意义。方法:采用流式细胞术分别检测38例妊娠期高血压病患者(妊娠期高血压病组)、30例正常妊娠晚期孕妇(正常晚孕组)和30例正常非孕妇女(正常非孕组)的血小板-中性粒细胞聚集体(PNA)和血小板-单核细胞聚集体(PMA)表达百分率。结果:妊娠期高血压病组PNA(60.3413.25)%、PMA(67.9211.22)%均明显高于正常晚孕组PNA(49.0515.26)%、PMA(60.3112.81)%和正常非孕组PNA(45.2713.69)%、PMA(47.8213.01)%,差异有显著性和非常显著性意义(P<0.05和P<0.01)。正常晚孕组PMA表达百分率明显高于正常非孕组,差异有非常显著性意义(P<0.01)。正常晚孕组PNA表达百分率与正常非孕组比较,差异无统计学意义(P>0.05)。结论:妊娠期高血压病患者存在血管内皮细胞损伤,血小板高度活化,聚集功能明显增强,血液呈血栓前或血栓状态。检测PNA、PMA的表达百分率对其病情观察、诊断和预防血栓形成有一定的临床意义。Objective To investigate the percentage change in the expression and clinical significance of platelet-leukocyte aggregates（PLA） of patients with hypertensive disorder complicating pregnancy（HDCP）.Methods Detected the percentage change of platelet-neutrophil aggregates（PNA） and platelet-monocyte aggregates（PMA） of 38 patients with hypertensive disorder complicating pregnancy（HDCP group）,30 normal late pregnancy pregnant women（normal late pregnant group） and 30 normal non-pregnant women（normal non-pregnant group） by flow cytometry.Results The HDCP group PNA（60.34±13.25）%,PMA（67.92±11.22）% were significantly higher than the normal late pregnant group PNA（49.05±15.26）%,PMA（60.31±12.81）% and normal non-pregnant group PNA（45.27±13.69）%,PMA（47.82±13.01）% individually.The difference was significant and highly significant（P0.05 and P0.01）.The expression rate of normal late pregnant group PMA was prominently higher than one of the normal non-pregnant group.The difference was statistically significant（P0.01）.Comparing the expression percentage of the PNA in normal late pregnant group and non-pregnant group,while the difference was minor of statistically significant（P0.05）.Conclusion The patients with HDCP have vascular endothelial cell injured,a high degree of platelet activation and aggregation increased notably before the blood clotted,or thrombosis was the state.Detecting the expression percentage of PNA and PMA has necessarily clinical significance to observe their condition,diagnose and prevent thrombosis.
