血清PCT和CRP对社区获得性肺炎的诊断价值

摘　　要：目的:探讨血清降钙素原(procalcitonin,PCT)和C-反应蛋白(CRP)在社区获得性肺炎(CAP)的诊断价值。方法:选择2009年9月～12月住院的CAP患者56例,健康对照组30例,所有对象均抽取静脉血,离心后留取血清置-20,采用免疫化学发光法测降钙素原,免疫散射比浊法测CRP。结果:56例CAP患者中,PCT≥0.15μg/L者47例,阳性率为83.92%;CRP≥10mg/L者35例,阳性率为62.5%;PCT和CRP的敏感性分别为83.92%、62.5%;PCT及CRP的特异性分别为100%、80.0%,PCT和CRP在CAP组较健康对照组明显升高,均有统计学差异(P<0.05);在CAP组内PCT的阳性率、敏感性、特异性均高于CRP,有统计学差异(P<0.05)。结论:PCT、CRP测定对CAP有一定的诊断价值,在阳性率、敏感性、特异性上,PCT均优于CRP。Objective To inverstigate the diagnostic value of measurement of serum PCT,CRP levels in patients with community acquired pneumonia（CAP）.Methods Subjects during Oct-Dec 2009,were divided into two observed groups（community acquired pneumonia group 56 patients）,and control group（30 healthy people）.All serum samples were centrifugaled and stored-20℃ for the measurements of PCT（by Brahms PCT-Q）,CRP（by using immunity-turbidity method）.Results Serum PCT level≥0.15μg/L in 47 cases of 56 cases with CAP,the positive rate is 83.92%;serum CRP level≥10mg/L in 35 cases of 56 cases with CAP,the positive rate of CRP is 62.5%;WBC count10×109/L in 17cases of 56 cases with CAP,the positive rate of CRP is 30.3%.For PCT,CRP,WBC count,the sentivities were 83.92%,62.5%,and 30.3%,separately.The specifitics were 100%,80%,and 96.6% for CAP diagnosis.For PCT,CRP,WBC count,neutrophil proportion,the statistic differences were obvious（all P0.01） between community acquired pneumonia group and control group.For positive rate,sentivities,specifitics,the statistic differences were obvious（all P0.05） between PCT and CRP in community acquired pneumonia group and PCT is better than CRP.Conclusion The measurements of the PCT,CRP levels is helpful for diagnosing community acquired pneumonia.The PCT is better than CRP in sentivities,specifitics and positive rate.
